About the Role
As an Oncology Nurse Navigator (ONN), you will serve as a consistent care coordinator for patients and their families, guiding them through their continuum of care with compassion and expertise. You’ll play a critical role in assessing patient needs, reducing barriers to care, and improving outcomes by facilitating communication, providing education, and advocating for quality services.
What We’re Looking For

Education: Graduate of an accredited nursing program.

Licensure: Current license to practice as a professional nurse in the state where work is performed or a compact license. BLS certification required for all new hires.

Experience: At least one year of nursing experience.

Preferred Qualifications:
- Bachelor’s degree in Nursing
- Outpatient nursing and scheduling experience
- OCN certification (must be obtained within two years of onboarding)

Skills:
- Strong communication and problem-solving abilities
- Knowledge of healthcare resources and oncology care
- Excellent organizational and computer skills
- Sensitivity, compassion, and ability to build collaborative relationships
What You’ll Do
- Coordinate patient care through assessment, planning, facilitation, and advocacy to ensure timely access to quality care.
- Provide education and anticipatory guidance to patients, families, and caregivers to reduce healthcare barriers and improve experiences.
- Track patient progress, outcomes, and care plans, collaborating with oncologists to make necessary adjustments.
- Manage scheduling of appointments, referrals to specialists, and coordination with external agencies to enhance patient care.
- Educate patients on symptom management, preventative care, and individualized care plans to reduce unnecessary admissions and over-utilization of resources.
- Perform clinical procedures within the scope of nursing practice and support patients through telephone consultations and follow-ups.
Physical Requirements
- Medium physical activity, including lifting up to 50 lbs. and frequent standing, walking, and reaching.
- Frequent patient contact and exposure to body fluids and hazardous materials.
- Requires close visual acuity, good balance, and coordination.
