About the Role

As an Oncology Nurse Navigator (ONN), you will serve as a consistent care coordinator for patients and their families, guiding them through their continuum of care with compassion and expertise. You’ll play a critical role in assessing patient needs, reducing barriers to care, and improving outcomes by facilitating communication, providing education, and advocating for quality services.

What We’re Looking For

Education: Graduate of an accredited nursing program.
Licensure: Current license to practice as a professional nurse in the state where work is performed or a compact license. BLS certification required for all new hires.
Experience: At least one year of nursing experience.
Preferred Qualifications:
- Bachelor’s degree in Nursing
- Outpatient nursing and scheduling experience
- OCN certification (must be obtained within two years of onboarding)
Skills:
- Strong communication and problem-solving abilities
- Knowledge of healthcare resources and oncology care
- Excellent organizational and computer skills
- Sensitivity, compassion, and ability to build collaborative relationships

What You’ll Do

Coordinate patient care through assessment, planning, facilitation, and advocacy to ensure timely access to quality care.
Provide education and anticipatory guidance to patients, families, and caregivers to reduce healthcare barriers and improve experiences.
Track patient progress, outcomes, and care plans, collaborating with oncologists to make necessary adjustments.
Manage scheduling of appointments, referrals to specialists, and coordination with external agencies to enhance patient care.
Educate patients on symptom management, preventative care, and individualized care plans to reduce unnecessary admissions and over-utilization of resources.
Perform clinical procedures within the scope of nursing practice and support patients through telephone consultations and follow-ups.

Physical Requirements

Medium physical activity, including lifting up to 50 lbs. and frequent standing, walking, and reaching.
Frequent patient contact and exposure to body fluids and hazardous materials.
Requires close visual acuity, good balance, and coordination.
