Position Overview
The Value-Based Care Manager is responsible for the healthcare management of oncology patients participating in value-based care programs. This role works closely with patients, families, and caregivers to match patient needs with appropriate services, maintain and update patient records, review clinical diagnoses and treatments, and ensure compliance with required practice redesign elements. The Care Manager serves as a key resource for patients at high risk for complications, helping to coordinate care, reduce barriers, and improve outcomes in alignment with West Cancer Center’s mission and values.
Key Responsibilities

Serve as a central point of contact for high-risk patients to manage care effectively and reduce redundancy.

Employ a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet health needs through communication and available resources.

Collaborate with the patient’s primary team, including nurse navigators, to ensure seamless coordination of care.

Assess needs of patients, families, and caregivers, providing anticipatory guidance and education.

Provide appropriate clinical actions within the scope of nursing practice.

Inform patients regarding preventative care practices and reinforce individualized care plans to prevent avoidable admissions or over-utilization of resources.

Track patient progress and outcomes, adjusting care plans as needed in collaboration with oncologists.

Coordinate appointments, physician communication, outside agency contacts, and supportive services.

Assist patients in managing health conditions, prioritizing care, and preventing complications.

Support transitions of care from inpatient to outpatient settings, ensuring medication management and scheduling of follow-up appointments.

Ensure staging and other clinical information is documented in collaboration with provider clinic staff.
Qualifications
Education & Experience

Required: Bachelor’s degree in Nursing.

Required: At least three (3) years of nursing experience.

Required: Current RN licensure in the state of practice.

Preferred: Master’s degree.

Preferred: Case management experience.
Skills & Abilities

Strong knowledge of healthcare management resources to match patient needs with appropriate services.

Excellent verbal and written communication skills, with strong conflict resolution and problem-solving abilities.

Proficiency in Microsoft Office applications and understanding of patient privacy/security regulations.

Ability to work independently, exercise judgment, and manage multiple priorities.

Strong organizational and workflow management skills.

Ability to build collaborative relationships with physicians, patients, families, and staff.
