Case Manager for High-Risk Youth
WestCoast Children’s Clinic, located in Oakland, California, is a non-profit community psychology clinic that provides mental health services to Bay Area children, youth and families. The C-Change Case Manager works in collaboration with WestCoast clinicians and parents, social workers, probation officers, teachers, and other adults in youths’ lives to provide advocacy and linkage to support clients' mental functioning. These services may include coordinating with education and vocational programs, housing, primary health, psychiatry, benefits and other independent living skills services. The C-Change Case Manager must have strong advocacy skills, demonstrate flexibility, utilize a strengths-based approach, and have demonstrated experience working with high needs youth.
How the C-Change Case Manager makes an impact:
- Provides advocacy, targeted case management, and community based rehabilitation services to transition-age youth.
- Assists youth in identifying and utilizing community services and resources to address their needs, including: housing, medical/physical health, education, employment, independent living skills, advocacy, placement stability, and age-appropriate enrichment activities in Alameda and SF county.
- Provide risk assessments, crisis intervention, stabilization, safety plans, and drop-in counseling (as needed).
- Connects youth with legal representation, as needed and appropriate, and supports youth in court appearances.
- Provides psycho-education about sexual exploitation and advocacy services for clients.
- Assists youth with accessing SSI and/or regional center services, appropriate housing resources including acute and structured residential treatment options as appropriate.
- Follow up with youth when incarcerated or hospitalized and transitioning to placements.
- Assists youth with obtaining and keeping Medi-Cal.
- Complies with policies, procedures, documentation and paperwork for EPSDT Medi-Cal billing and meets billing requirements as set by the agency.
- Provides services in the community, for which travel by car is necessary.
- Demonstrates professional and ethical conduct at all times as a representative of the WestCoast community.

Minimum Requirements:
- At least two years of experience working with youth in systems-involved settings.
- Bachelor’s degree in Psychology, Criminal Justice, Social Work, or related field preferred.
- Bilingual in Spanish preferred.
- Experience in case management and rehabilitation services.
- Proven experience with adolescents with dual diagnoses, high mental health needs, and/or Sexually Exploited Minors.
- Knowledge of Alameda and SF County resources for TAY youth.
- Experience working in diverse settings (homes, schools, neighborhoods, agencies).
- Skilled in working with clients with complex trauma and attachment issues.
- Strong case management skills (assessment, crisis intervention, supervision, documentation).
- Cultural humility and awareness of race, class, and ethnicity issues.
- Strong organizational, time management, and efficiency skills.
- Ability to work independently and within multidisciplinary teams.
- Knowledge of Medi-Cal reimbursement and billing procedures, or ability to learn.
- Ability to travel by car to meet with clients, generally within a 90 mile radius of WCC's Oakland office. Alternative forms of transportation may be accommodated based on the needs of the program and clients. Employees must have a clean CA DMV record. When driving for WCC, current vehicle insurance is required.
Position details and compensation:
This is a full time, hourly position. Salary: $65,000 per year $71,500 per year for bilingual Spanish). Includes competitive benefits package, 403(b) plan, and performance-based compensation.
