Care Coordinator and Group Facilitator

WestCoast Children’s Clinic, located in Oakland, California, is a non-profit community psychology clinic that provides mental health services to Bay Area children, youth and families. The Team-Based model of care program named EMPOWR (EMbracing, Progress and Opportunities, We Rise) aims to support transition aged youth (TAY, 16-24) attain mental health support, housing stability, employment and education gains, and community connections. The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients.

The Intensive Care Coordinator (ICC) provides support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth’s natural supports together. They lead the facilitation of Child and Family Team (CFT) meetings, ensuring that youth and their chosen supports—natural supports (e.g., friends, extended family, mentors) and formal system partners (e.g., child welfare, education, juvenile justice)—are engaged in a collaborative planning process. The ICC helps coordinate, monitor, and support the implementation of individualized CFT plans, ensuring services are aligned with the youth’s goals and cultural context.

The position draws on lived experience to provide support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth’s natural supports together. Because of their lived experience, they help create a connection with family and other close contacts, and offer a unique perspective to the rest of the team. 

How you'll make an impact:

  • Provide Intensive Care Coordination (ICC) services as a core member of the EMPOWR team, working collaboratively across disciplines to support shared caseloads and participant progress.
  • Partner with youth, families, natural supports, and formal system partners to facilitate a strength-based, culturally responsive, and youth-centered planning process.
  • Schedule, organize, and lead regular Child and Family Team (CFT) meetings to promote collaborative decision-making, care coordination, and goal alignment. (Facilitation training provided.)
  • Conduct short-term needs and strengths assessments to guide care planning and empower youth voice and choice.
  • Provide stabilization and immediate support to foster youth at a 24-hour drop-in center as they prepare for transition to foster placements or other supportive settings.
  • Complete Medi-Cal-compliant documentation within 48 hours of service delivery, maintaining accuracy and alignment with program, legal, and funding requirements.
  • Uphold the mission and values of the agency by demonstrating cultural humility, maintaining professional boundaries, and fulfilling mandated reporting responsibilities.
  • Engage consistently in team meetings, group and individual supervision, and cross-team collaboration to ensure aligned and integrated service delivery.
  • Support the team in tracking care coordination efforts and contributing to shared accountability for participant outcomes.
  • Perform other duties as assigned by the supervisor or program director in support of youth, team function, and program development.

Qualifications:

  • Bachelor’s degree in Psychology, Criminal Justice, Social Work or related field preferred
  • Direct, personal experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources is highly desirable.
  • 2 years of experience working directly with young adults (TAY) and/or families from a mental health framework.
  • Facilitation experience and skills are highly desired
  • Ability to assess needs, strengths & provide crisis intervention and de-escalation
  • Knowledge of wraparound or team based care services
  • Passionate about working with young adults who have experienced complex trauma
  • Strong desire to learn about: Assessing, crisis intervention and de-escalation skills, foster care/child welfare systems, mental health systems, and Alameda County resources
  • Strong organizational, written & communication skills - A writing sample exercise will be provided during the interview process.
  • Values working on a team, cultural humility, compassion, inclusivity, self-reflection, ongoing personal growth, self-care and social justice.
  • Flexible schedule with ability to work evenings and occasional weekends as needed.
  • Ability to travel by car to meet with clients, generally within a 90 mile radius of the WCC office. Clean CA DMV record, current car insurance and a vehicle; travel is required

Competencies (skills, abilities, and knowledge):

  • Demonstrated experience working with children and families who may be involved in the child welfare system.
  • Case management skill – assessment/evaluation, crisis intervention, ability to use supervision effectively, ethical and legal responsibilities, high quality documentation.
  • Cultural humility - knowledge of issues of race, class and ethnicity.
  • Efficiency and time management - ability to be well-organized and complete timely documentation.
  • Collaboration and team functioning - able to work independently and collaboratively as part of a multidisciplinary team.
  • Ability to work with clients who have complex traumatic histories and other attachment difficulties.

Position details and compensation: This is a full time, hourly (non-exempt) position. Salary starting at $65,000 per year, $31.25 per hour (added 10% for bilingual Spanish) and includes performance-based compensation plan, competitive benefits package, and 403(b) plan.
