Position Summary:

The Social Services Coordinator will be responsible for providing comprehensive social, community, and financial resources to clients. This role requires a compassionate and organized individual who can effectively connect clients with the necessary services and support to improve their quality of life.
Key Responsibilities:
- Conduct initial assessments to identify clients' needs and eligibility for various services.
- Develop individualized service plans tailored to each client's unique circumstances.
- Provide clients with information on available social services, community programs, and financial resources.
- Assist clients in completing applications for benefits and services, such as housing assistance, healthcare, and financial aid.
- Build and maintain relationships with community organizations, service providers, and government agencies.
- Organize and facilitate community outreach events and workshops to raise awareness of available resources.
- Maintain detailed and accurate records of client interactions, service plans, and progress.
- Monitor and follow up with clients to ensure they are receiving the support they need and adjust service plans as necessary.
- Advocate on behalf of clients to ensure they receive fair and timely access to services.
- Provide emotional support and guidance to clients facing difficult situations or transitions.
- Work collaboratively with other team members, departments, and external partners to coordinate services and support for clients.
- Participate in regular team meetings and training sessions to stay updated on best practices and resources.
- Performs other duties as required.
Qualifications:
- Minimum of 2 years of experience in social services, case management, or a related field.
- Experience working with diverse populations and knowledge of community resources.
- Bachelor's degree in social work, Human Services, Psychology, or a related field preferred.
Physical Requirements:
- Office environment with normal business hours necessary to satisfactorily perform job functions.
- Stand/Sit – Must be able to remain in a stationary position up to 50% of the time.
- Walk – This position needs to occasionally move about inside the office to access file cabinets, meeting rooms, office machinery, etc.
- Use of hands/fingers to handle or feel – constantly operates a computer and other office machinery such as a copy machine/printer.
- Climbs stairs – May occasionally ascends/descends the stairs to get to other floors, cafeteria and parking area.
- Carry weight/lift - Occasionally moves supplies weighing up to 10 pounds for various presentation and event needs.
