Role Description 

The Engagement & Intake Specialist is a critical frontline role within Zócalo Health. This role blends community-based outreach and patient intake responsibilities to ensure new and prospective patients are welcomed into care, educated about available services, and successfully enrolled in programming. You will engage directly with patients through in-person and virtual settings, conduct intake assessments, and coordinate follow-up care with clinical and operational teams.

This position reports to the Community Engagement Manager. You will work primarily in your community, with some work-from-home responsibilities.

 

Key Responsibilities:

Outreach & Engagement

  • Conduct in-person outreach attempts for members identified via health plan eligibility lists.
  • Attend and represent Zócalo Health at community outreach events, community coalitions, and collaborative meetings.
  • Engage prospective patients, collect contact information, and educate them on Zócalo’s services and programming.
  • Make follow-up calls and send mailers to community event leads to encourage enrollment.

Intake & Onboarding

  • Complete scheduled intake appointments (virtual and in-person) to collect consents, demographic data, and clinical/social history.
  • Assist patients in completing forms and onboarding documentation.
  • Create care planning accounts and update the county-based case management tracker.
  • Schedule follow-up appointments with the assigned case manager and/or clinical team.

Documentation & Coordination

  • Accurately document outreach and intake activities in the EHR and internal tracking systems.
  • Convert leads to active patients using established workflows and tools.
  • Support documentation efforts for claims submission and assist in data collection/reporting.
  • Escalate barriers to care to appropriate teams to ensure timely intervention and resolution.

 

Qualifications

Language/Culture

  • Fluency (verbal and written) in English and Spanish.
  • Knowledgeable of Latino customs and cultural norms

Education

  • High school diploma or GED (minimum). 

Licenses/Certifications (CA only) 

  • Must possess a Community Health Worker certification, which included field experience as a requirement for completion -OR-
  • Demonstration of 2,000+ hours of CHW work (paid or volunteer) in the past three years and willingness to obtain a Community Health Worker certification within 18 months of hire date

Experience

  • 1–3 years of experience in healthcare, community outreach, case management, or a related field.
  • Prior experience in patient intake, outreach, or care coordination strongly preferred.
  • Flexible schedule, including ability to support occasional evening or weekend events.
  • Training in motivational interviewing. (preferred)

Complementary competencies and skills

  • Exceptional interpersonal skills and cultural competence; ability to build trust with diverse communities.
  • Detail-oriented and organized, with ability to manage multiple priorities.
  • Technologically savvy, with experience using EHRs (Athena experience preferred).
  • Comfortable working both independently and collaboratively across teams.
  • Well-known in and have strong ties to the local Latino community. (preferred)
  • Well versed in local resources to support SDOH needs. (preferred)

Flexible and able to travel to other communities

  • Willing to travel to support community events and in person patient appointments. 
  • Have reliable sources of transportation.
