Community Supports (CS) Program Manager
Akido is rebuilding the healthcare experience from the ground up. Through early interventions designed around social determinants of health (SDoH), world-class care focused on chronic disease, and human-centered technology, we believe we can build a healthcare model that allows for all patients to live their fullest lives. The Opportunity
At Akido Labs, we are redefining healthcare for underserved communities by addressing the medical and social needs of those most at risk. As a Community Supports (CS) Program Manager, you’ll play a pivotal role in leading housing navigation and support teams that deliver compassionate, effective care to members facing housing insecurity, mental illness, and complex medical conditions. Reporting to the Director of Operations, this position blends leadership, strategy, and hands-on management to ensure program excellence and measurable community impact.

What you’ll do
- Lead and supervise Housing Navigators and Housing Specialists, ensuring caseloads are balanced and productivity goals are achieved.
- Build a supportive and resilient team culture through regular coaching, one-on-ones, and burnout-prevention strategies.
- Oversee member care plans, ensuring quality, accuracy, and timely implementation.
- Fully train and onboard new staff within 4 weeks, ensuring full caseload management by week 6.
- Maintain a small personal caseload to stay connected to field operations and member needs.
- Collaborate with the Director of Operations to meet audit requirements, performance metrics, and compliance standards.
- Manage program logistics including vehicles, vendor relationships, inventory, and operational budgets.
- Drive continuous quality improvement initiatives and identify innovative solutions to operational challenges.
- Facilitate daily huddles, weekly data reviews, and monthly team presentations highlighting impact and outcomes.
- Represent the program in Health Plan Operations meetings and build partnerships with community and housing agencies.
Who you are
- Must-Have Skills and Experience
- Experience in human services, property management, leasing, or a related field.
- Knowledge of landlord-tenant law, fair housing standards, and rental contracts.
- Strong understanding of Medi-Cal populations, including individuals experiencing homelessness, mental illness, or substance use disorders.
- Proven ability to manage staff, oversee caseloads, and meet performance goals.
- Excellent organizational, communication, and conflict-resolution skills.
- Experience with compliance, documentation systems, and data-driven decision-making.
Additional (Preferred) Qualifications
- Advanced training in a related field (e.g., Master’s in Social Work, SUD certification).
- Demonstrated leadership in healthcare or social services.
- Expertise in motivational interviewing, harm reduction, Housing First, and trauma-informed care.
- Valid driver’s license, automobile insurance, and reliable transportation.

Physical Demands: Mostly sedentary work duties require exerting up to thirty pounds of force occasionally and/or small amounts of force frequently. Sedentary work typically involves sitting most of the time but may involve walking or standing for brief periods.
Salary range
$80,000 - $100,000 USD
