Eliot’s Outpatient Clinic is seeking a Service Coordinator to provide Intensive Community Care and Support (ICCS). Eliot's Outpatient Clinic provides evidence-based evaluation, counseling, and medication management for children, families and adults with mental health and substance use needs. 

In this role, the Service Coordinator will develop and implement goal-oriented service plans, provide side-by-side interventions, and oversee the day-to-day needs and overall service delivery of consumers in the program. The ideal candidate must have a strong commitment to mental health recovery and the ability to establish supportive relationships with persons served.

Responsibilities:

  • Engage and contact newly referred consumers within 24 hours of referral complete assessment(s) and initiate service planning.
  • Participate in all service planning and interagency Planning Meetings both internally and externally as directed.
  • Provide side by side interventions to consumers that focus on problem solving, skills training, modeling behavior optimism and encouragement.
  • Develop and implement a goal-oriented service plan in accordance with Quality Improvement Standards, sound practice, and in collaboration with consumer and treatment team.
  • Facilitate consumer treatment compliance and community stability through use of aggressive outreach, education, support, hands-on assistance and teaching consistent with identified service provision goals.
  • Utilize motivational interviewing, trauma informed care and other best practices with all aspects of service delivery.

Qualifications:

  • Requires a bachelor’s degree in psychology, social work or related field
  • Experience working with individuals with mental illness and co-occurring medical and/or substance use disorders.
  • Valid driver's license and reliable transportation (required for the transportation of clients to and from appointments and activities as needed)

Schedule: Monday - Friday, 9:00am-5:00pm

Annual Salary

$50,000 - $52,000 USD
