Oncology Social Worker
Professional Social Worker to support oncology team families, and caregivers to help overcome healthcare system barriers. Working with the clinical care team, this position will support financial and emotional needs of our Members, while striving to maintain optimal outcomes for the Member and the plan.
Location: Dallas, Texas | Onsite Full Time
Responsibilities:
- Working in a team environment with the nursing staff and care advocates to provide a seamless care experience for Members
- Research and coordinate financial resources for the Member, both locally and nationally
- Provide emotional support to Members while they are navigating through their cancer diagnosis, treatment and survivorship
- Assist patient and Clinical Care team with navigating and maximizing benefits
- Assists patients or their caregivers with developing scope of services with clearly stated goals and objectives.
- Providing education and support to the patient and family
- Identifying special needs of the patient and delegating to appropriate support staff
- Strong collaborative relationships with Clinical Care Team
- Meeting with patient and caregivers to review their concerns, both financial and emotionally
- Provide education, resources, and referrals to local and national resources.
- Provides emotional support, especially during difficult decision-making periods.
- Demonstrates the knowledge, skill, and coordination to provide support and guidance to the cancer patient from screening to survivorship.
- Facilitate shared decision making: Shared decision making involves ensuring that patients are fully informed of risks and benefits of treatment options and that their values and
- Helping patient and families understand their medical conditions and treatment options.
- Coordinating care by assisting with appointments, including transitioning care outside of their state or other facilities.
- Promote advance care planning: The LCSW can promote advance care planning by encouraging patients to talk about their treatment wishes and overall goals of care. The LCSW can help patients formulate questions to discuss with their healthcare providers.
- Support palliative care: The LCSW can support patients’ use of palliative care services by assessing for late and long-term side effects and other physical barriers to patients’ quality of life. The LCSW can also be instrumental in helping patients understand the difference between palliative care and hospice.
- Inform to ONN (Oncology Nurse Navigator) any concern regarding a patient needs
- Functions in an organized and time conscious manner. Partners with patients, families and community resources to provide well-coordinated, timely, compassionate, exemplary, interdisciplinary care.
- Communicates with all members of the healthcare team, as appropriate about patient/family needs and concerns.
Requirements:
- BSW/MSW/LCSW
- Graduate of accredited Social Work Program
- Licensure in the state of Texas, compact license preferred
- Overall computer literacy with proficiency in Windows-based software and Microsoft Office.
- Experience working with Oncology patients is preferred
- Experience locating local resources and financial resources
- Knowledge of insurance payors, systems and reading insurance benefits preferred
