Reports To: Clinical Director / Director of Patient Care Services
Location: Inland Empire, CA
Job Type: Full-Time


Job Summary

We are seeking a dedicated and experienced Clinical Supervisor (RN) / Nursing Supervisor to join our team. In this leadership role, you will be responsible for overseeing the coordination, implementation, and evaluation of clinical care services in both hospice and home health settings. You will supervise a team of healthcare professionals, ensure compliance with care standards, and promote continuous quality improvement across all services.


Key Responsibilities

  • Manage patient care coordination and oversee clinical staff for both hospice and home care services.

  • Review patient referrals, assign appropriate personnel, and ensure timely admissions.

  • Conduct home visits and supervise care delivery to ensure compliance with clinical protocols and regulations.

  • Collaborate with the Medical Director and interdisciplinary team to determine eligibility and develop individualized care plans.

  • Guide and mentor clinicians in setting therapeutic goals, prioritizing patient needs, and enhancing care delivery.

  • Participate in case conferences to ensure continuity and coordination of care.

  • Conduct quarterly chart audits and provide performance feedback to the care team.

  • Assist in recruiting, hiring, training, and orienting new clinical staff.

  • Contribute to in-service training programs and staff education.

  • Participate in performance improvement initiatives and assist with monthly billing workflow.

  • Engage in community outreach and public relations efforts to promote the hospice’s role in healthcare delivery.


Qualifications

  • Registered Nurse (RN) with current license to practice in California.

  • Graduate of an accredited nursing school; BSN preferred.

  • Minimum of 1 year of nursing experience within the last 3 years; hospice or home care experience preferred.

  • Minimum of 2 years’ experience in a supervisory or management role.

  • Excellent clinical assessment, nursing judgment, and communication skills.

  • In-depth understanding of hospice philosophy, end-of-life care, and pain/symptom management.

  • Must have a valid driver’s license, reliable transportation, and current auto insurance.

  • CPR certification required and must be maintained.
