Givens Estates, a nonprofit life plan community in Asheville NC, and Givens Highland Farms in Black Mountain, is hiring a full-time LPN Team Lead to help support our Home First Department. As an LPN Team Lead for Givens Home First, you will have the opportunity to support a growing program that helps community members live a healthy, happy, and safe life at home. You will establish and maintain communication with clients and their family members regarding their care, lead the intake process for new Home Care clients, and supervise Home Care Companion and In-Home Aide level clients.
What you'll do:
- Establish and maintain communication with clients and their family members/POA's regarding their care
- Process referrals for Home Care within one business day
- Develop and maintain strong relationships with referral sources
- Effectively communicate with the RN Assessment Nurse to ensure appropriate client assignments occur
- Supervise a team of In-Home Aides/Companions
- Support the Home Care program with initial client intake
- Directly communicate with the Administrative Coordinator regarding system problems, repairs, and troubleshooting as needed
- Participate in ongoing education and training offered by Givens and/or outside resources as required
- Ensure any observed or suspected abuse, neglect, or exploitation is reported to proper authorities as mandated as a health professional
- Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor
What you'll need:
- Must be currently licensed in the State of North Carolina as an LPN
- No evidence of exclusion from the OIG Registry
- Previous experience in home care, home health, geriatrics or long-term care is desired
- Previous experience leading a team, and experience supporting as an administrative assistant is desired
- Strong computer and interpersonal skills
- Active CPR (cardiopulmonary resuscitation) certificate or willingness to obtain within six months of joining Givens
Compensation is $33.00 - $35.00 per hour plus shift differentials, in addition to our comprehensive benefits package!
