Role Scope: The Growth team focuses on amplifying the impact of the PACE model of care by educating and expanding access to the program. Our team manages the end-to-end process of identifying, attracting, engaging with, assessing, and enrolling new participants.
The Manager, Growth Strategy & Operations, will help launch new markets and strengthen the operational backbone that supports the team’s mission. Reporting into Growth Strategy & Ops leadership, this role pairs on-the-ground launch support with the development of repeatable playbooks, enablement resources, and change management practices that allow Growth Outreach & Enrollment to scale smoothly as Habitat Health expands across markets.
Location: Candidates based in San Francisco, Sacramento, or Los Angeles, California are preferred; candidates located elsewhere in the Western US (Pacific or Mountain time zone) will also be considered

Core Responsibilities & Expectations for the Role
- Support new market launches on the ground, working with local teams and cross-functional partners to stand up Growth operations
- Develop and maintain the new market launch playbook into a repeatable framework and project plan that accelerates and de-risks each subsequent launch in both CA and in new states Habitat Health enters.
- Drive operational enablement across the Growth team, including supporting new program rollouts and resource and workflow optimization.
- Gather and implement feedback from Growth Operations on new programs and operational workflows, translating frontline input into concrete process improvements and updated guidance.
- Support regular reporting and performance tracking, monitor how new processes and launches are performing, surface opportunities for iteration, and help drive continuous improvement.
- Collaborate across the organization - including Clinical Operations, Operations, Marketing, and partner teams - to align launch and enablement work with broader growth priorities and the participant journey.
- Facilitate cross-functional and external stakeholder engagement to align on and drive progress against key initiatives.
Required Qualifications:
- A Bachelor’s degree or 3-5 years of relevant experience in health care delivery at a high-growth startup, operations or program management, or consulting for health care payers & providers.
- Demonstrated ability to be a self-starter, with comfort operating in ambiguity, an aptitude for building structure where none exists, and a bias to action.
- Strong project management and organizational skills, with the ability to manage multiple workstreams across functions and locations.
- Strong interpersonal and written/oral communication skills; high EQ.
- A practical problem solver who can connect strategy to daily execution and thrives in a fast-paced, evolving environment.
- Willingness to work in a hybrid work environment, with travel to Habitat Health centers and new market sites for launches and stabilization periods.

Compensation:
The anticipated base salary range for this position is $135,000 - $154,000. This position is also eligible for a target annual bonus of 10% and may be eligible for additional incentive compensation, as applicable.
The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.

At-Will Employment
Employment with Habitat Health is at-will. This means that either the employee or the employer may terminate the employment relationship at any time, with or without cause, and with or without notice, subject to applicable law.
