Outreach & Enrollment Trainer

California ( LA, Sacramento, Oakland)

Habitat Health empowers older adults to experience more good days in their homes and communities. Through the Program of All-Inclusive Care for the Elderly (PACE), we provide comprehensive medical care along with support for daily needs such as meals, transportation, and inhome assistance. We deliver coordinated clinical and social care in our centers and directly in participants’ homes, creating a fully integrated experience that brings peace of mind and a true sense of belonging. As we expand our scalable, affordable PACE model to meet the growing and complex needs of aging populations, our missiondriven care teams continue to help participants live well on their own terms.

Habitat Health is supported by leading healthcare organizations and investors including New Enterprise Associates, Kaiser Permanente, and Town Hall Ventures. We are entering a period of significant growth and are looking for exceptional teammates to help us scale a better model of care for older adults. To learn more, visit www.habitathealth.com.

Estimated Start Date:  9/1/26

Role ScopeThe Growth team focuses on amplifying the impact of the PACE model of care by educating and expanding access to the program. Our Outreach & Enrollment (O&E) teams are the front line of that effort - finding, engaging, and enrolling new participants across our markets.

The Outreach & Enrollment Trainer is critical in making these teams effective, efficient, and compliant. You’ll own the training for new O&E hires and how the broader team continuously improves over time — from training plans and competency checklists, onsite trainings, call and field coaching, new market cohorts, and self-service modules. This is a sales-oriented training role: the goal is to develop confident, compliant reps who can engage prospects to generate and convert referrals. You’ll work hand-in-hand with O&E Supervisors and carry a dotted line to Learning & Development.

Core Responsibilities & Expectations for the Role

  • Build and run 30/60/90-day training plans for new O&E hires in collaboration with Outreach & Enrollment supervisors, with clear milestones for each phase of ramp.
  • Create and maintain competency checklists that define what “good” looks like by role and confirm reps are ready before they work independently.
  • Deliver onsite training at our centers, partnering with O&E Supervisors to tailor it to each team and market.
  • Coordinate shadowing and call/field auditing alongside Supervisors, pairing new hires with strong reps and giving structured feedback. Lead new center cohort training, getting whole teams ready to conduct job duties on day one.
  • Audit O&E teams on a regular basis to check for consistency, quality, and compliance, and to find where reps need more support.
  • Identify and synthesize capability strengths and gaps both within and across centers to inform relevant interventions
  • Design role-specific, market-specific, and general O&E training programs, from outreach and prospecting through enrollment conversion.
  • Develop self-service and more sustainable training modules so learning scales without always needing a live trainer.
  • Own ongoing L&D for the O&E team, including refreshers, upskilling, and reinforcing best practices well past initial training.
  • Maintain training that reflects all CMS, DHCS, and Habitat Health marketing and compliance requirements.

Qualifications

  • 4+ years of experience in sales, outreach, enrollment, or customer engagement, including time spent training, coaching, or onboarding others.
  • Experience building training plans, competency frameworks, training materials, or job aids.
  • Confident facilitator, comfortable leading onsite and virtual training for new and tenured reps.
  • Self-starter who can build structure where none exists and operate in a fast-paced, evolving environment.
  • Strong interpersonal and written/oral communication skills; high EQ.
  • Willingness to work in a hybrid environment, with regular travel to Habitat Health centers and new markets.
  • Proof of medical clearance for communicable diseases, including a TB test.
  • Proof of all immunizations are up to date.
  • Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.

Preferred Qualifications

  • Bachelor’s degree is preferred.
  • Experience in Medicare/Medi-Cal outreach, telephonic enrollment, healthcare sales, or a contact center environment.
  • Familiarity with TPMO/CMS marketing compliance requirements.
  • Experience with L&D tools, LMS platforms, or building self-service training content.
  • Familiarity with the PACE program or other elderly care organizations.
  • Bilingual: Spanish strongly preferred.
