The Opportunity:

Join us in revolutionizing healthcare for rural America! The Provider Success Consultant is a key relationship leader responsible for developing and maintaining strong partnerships with physicians and clinics in Michigan. This role blends consultative engagement, lead generation, implementation support, and ongoing collaboration to help providers improve outcomes for their patient populations.

This individual will establish and build relationships with providers, identifying opportunities that generate interest in Homeward’s solutions and expand the pipeline of engaged practices. Successful candidates can: serve as a trusted advisor, help providers explore solutions that enhance patient engagement, surface opportunities to improve quality measures, coordinate documentation and data-sharing, and identify prospects for expanded partnerships. In order to do this, they will work with internal Homeward teams to qualify leads, advance opportunities, and ensure providers are prepared to succeed in value-based care models.

What You’ll Do:

  • Engage Providers & Generate Leads:
    • Introduce practices and hospitals to Homeward and articulate Homeward’s solutions and collaborative opportunities
    • Act as a consultant for practices to identify provider needs and position Homeward as a partner
    • Identify, qualify, and document provider leads in our CRM to build a healthy pipeline of prospective partners
    • Support the development of outreach strategies to expand provider awareness and interest
  • Implement Solutions:
    • Partner with internal teams to support early implementation efforts when leads transition to active partnerships
    • Coordinate with internal and external teams to ensure complete and timely project execution 
  • Manage Relationships:
    • Conduct regular in-person visits, to deliver results, understand the practices’ evolving needs, and real-time support/service recovery
    • Help practices interpret and understand their role in achieving VBC outcomes on Homeward’s at-risk patients
    • Capture and communicate provider needs, objections and feedback for internal development efforts
  • Drive Performance Improvement:
    • Regularly monitor provider performance dashboards and reports, and know how to interpret and explain those to provider partners
    • Assess providers’ capabilities and compose a plan to bring them along a path to enhance their reach and impact
  • Collaborate Internally:
    • Work closely with clinical, analytics, and operations teams to deploy those resources to further provider partnerships
    • Maintain an awareness of market developments, and act as a local expert on the provider environment for Homeward 
    • Translate frontline provider needs into recommendations for strategy and product development 

What You Bring:

  • 3–7 years’ experience in provider relations, clinical outreach or value-based care delivery
  • 2+ years of experience in lead generation, business development, or liaison/representative roles with physician practices and clinics
  • Functional understanding of provider practice operations, revenue cycle, and value-based care
  • Excellent presence and ability to build/maintain relationships
  • Strong independence, autonomy, and initiative  
  • Ability and desire to travel regularly to rural locations across Michigan (residence in Michigan Lower Peninsula strongly preferred)
  • Bachelor’s degree required; MPH, MHA, RN, or equivalent experience preferred

The base salary range for this position is $63,000 - $96,000. Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, location, competencies and experience. Base pay is one part of the Total Package that is provided to compensate and recognize employees for their work at Homeward Health. This role is eligible for an annual bonus, stock options, as well as a comprehensive benefits package.
