
Manager, Community Health ED Navigator

The primary location for this role remote, the expected schedule requirement is 8:00am - 05:00pm (Monday-Friday).
What You’ll Do
Reporting to the Director, BH Acute Care, the CHN ED Navigator Manager will oversee the Community Health Navigator team responsible for our medical transition of care programming for emergency room discharges. This role is largely administrative in that it oversees a non-clinical team who are completing Transition of Care visits. However, it is expected that this role will provide clinical oversight of that team, ensuring that patients are properly escalated and to review any pertinent medical concerns. The role may also be patient-facing for delivery of service/escalations as is needed.
- Manage a team of non-clinical roles focused on transitions of care.
- Uphold team members to Imagine specific policies, clinical programming requirements, and utilization targets.
- Provide 1:1 supervision to team members to review both administrative and clinical topics.
- Organize and lead departmental team meetings inclusive of attendance, agenda, and meeting minutes
- Manage acute escalations from team members and provide guidance on next steps.
- Take escalation TOC visits directed from the CHN ED Navigators.
- Evaluate care team performance quarterly and hold members accountable for KPIs and SLAs.
- Work collaboratively with all other leadership roles to respond to feedback, improve patient experience, and support the development of new programs and services.
- Act as SME for CHN ED Navigator questions. May also be a point of contact/SME for TOC RNs.
- Identify and participate in cross departmental quality improvement efforts.
- Identify and participate in strategic efforts to best support the service line's operational needs.
- Communicate progress, needs, and updates to leaders, escalate issues when necessary.
- Performs other duties as assigned.
What You Bring & How You Qualify

- You have at least 3 years of experience as a Registered Nurse in a high-volume acute care setting.
- Graduate with a Bachelor's of Nursing from an accredited university is required.
- At least one year of experience working with transitions of care population or as part of a team specifically focused on preventing inpatient/ED readmissions
- Experience managing medically complex pediatrics required.
- Ambulatory pediatric practice experience strongly desired.
- Tele-health experience a plus, especially in the virtual urgent care space
- Compact licensed RN in one of the following states (TX or FL) is required.
- Maintenance of professional credentialing and CME standards.
- Proficiency in Spanish required.

The salary for this role is $100,000 - $130,000 in addition to competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable). When determining compensation, we analyze and carefully consider several factors including job-related knowledge, skills and experience. These considerations may cause your compensation to vary.
