Position: Care Coordinator

Job Summary: Supporting individuals dealing with substance abuse or addiction issues as they transition into sober
living environments.

Duties and Responsibilities:

  • Provide ongoing case management services to residents, including regular check-ins, goal setting, and progress monitoring. 
  • Serve as a liaison between the recovery housing program and treatment providers, facilitating communication and information sharing regarding case management services.
  • Coordinate and schedule appointments with treatment providers, ensuring residents have access to necessary services and follow-up care.
  • Assist residents in accessing community resources and support services, such as employment assistance,  educational programs, and legal services.
  • Advocate for residents' needs and rights, ensuring they receive appropriate and quality care.
  • Conduct regular meetings with residents to address common challenges, provide education, and promote community-building.
  • Collaborate with the recovery housing program staff to ensure a safe and supportive living environment for residents.
  • Maintain accurate and up-to-date documentation of resident interactions, progress, and treatment plans.
  • Participate in team meetings and treatment team meetings to discuss resident progress and develop strategies for improved care.
  • Provide crisis intervention and support to residents in times of relapse or other emergencies.
  • Conduct medication counts at the request of clinical staff.
  • Coordinate discharge planning and support residents in transitioning to independent living or other appropriate levels of care.
  • Collaborate with external agencies, community organizations, and treatment providers to enhance the resources available to residents.

Required Qualifications: 

  • Minimum high school diploma/ E.D
  • Valid CDCA 
  • Knowledge of Electronic Health Records systems
  • Valid State of Ohio Driver's License and insurance
  • Must pass BCI check, all Corporate Compliance checks, and employment drug
  • Familiarity with computers and technology/ computer programs (Word, Excel, Outlook and Teams)
  • Must be 25 years or older

Schedule: 

Sunday - Thursday 4pm - 12am

Tuesday - Saturday 4pm - 12am 

(hours and days may vary depending on business need)

Location: 3025 W Broad Street Columbus, Ohio 43204

Compensation Range: $19.25 - $24.39
