Position: Care Coordinator, Recovery Housing

Job Summary: The Care Coordinator reports directly to the Recovery Housing Program Supervisor. The Care Coordinator plays a critical role in managing and coordinating case management services for individuals in recovery. This role involves helping residents access safe and supportive housing, assisting with progress being made toward recovery and treatment goals, and offering ongoing support to residents to ensure their stability and success. The Care Coordinator works closely with residents, staff, and community partners to address housing needs and any challenges to recovery.

As a Care Coordinator, you will have distinct housing and clinical roles and different duties applicable to each role. While working in your housing capacity, you are expected to only fulfill your housing duties and complete housing tasks. While working in a clinical capacity, you are expected to only fulfill your case management duties and complete clinical tasks.

Duties and Responsibilities:

Housing Duties:

  • Welcome new residents into the home, clearly explaining all program rules and expectations.
  • Serve as a liaison between the housing program and external service providers, facilitating effective communication and information sharing.
  • Lead weekly meetings with residents to address challenges, share information, foster a sense of community, and discuss their recovery plans.
  • Collaborate with housing program staff to maintain a safe and supportive living environment.
  • Be available to residents who need additional support or have questions.
  • Assist with medication counts, as needed.
  • Being available, as needed, to cover shifts overnight and on weekends to ensure paid staff are on-site in Level 3 recovery housing when residents are present.
  • Support residents in planning and preparing for their transition to independent living or other suitable housing options.
  • Lead by example, demonstrating prosocial behavior and recovery principles in your daily interactions.
  • Transport residents, travel between multiple locations

Clinical Duties:

  • Support residents to gain access to necessary medical, social, educational, vocational, recreational, and criminal justice services.
  • Help residents obtain financial assistance, job assistance, housing assistance, food, and clothing.
  • Coordinate client assessments and treatment planning.
  • Facilitate linkages to community resources and support, including self-help support groups.
  • Assist residents with increasing their social support networks with friends, family members, and/or organizations.
  • Assist residents perform daily living activities.
  • Complete documentation accurately and in a timely manner, as required by LRH policies, applicable payor requirements, and applicable laws.
  • Advocate for residents' needs, ensuring they receive high-quality care and support.
  • Provide residents with assistance during crises or emergencies, including relapse.
  • Participate in team meetings to review residents’ progress and develop strategies for enhanced support.
  • Participate in community activities that uphold the professional image of Lighthouse.

Required Experience/Certifications:

  • CDCA certification required
  • Valid driver’s license, proof of auto insurance, and access to a reliable personal vehicle
  • Must pass BCI check, all Corporate Compliance checks, and employment drug screen
  • Age 25 years or older

Preferred Experience/Certifications:

  • Bachelor's degree in social work, psychology, counseling, or a related field (or equivalent experience)

Compensation Range: $19.25 - $24.69

Schedule: Saturday - Wednesday 7am - 3pm or Wednesday - Sunday 8am - 4:30pm (hours and days may vary depending on business need)
