Responsibilities
The Care Navigation Services (CNS) Coordinator is responsible for knowing multiple areas of the Care Navigation team, including but not limited to: accurately and properly entering all new patient information after basic demographic information is entered by the Customer Service department; tracking and communicating a daily disposition for all patients in an acute or post-acute rehabilitation setting; preparing reports; and approving CCM and CPO minutes. Responsibilities are completed with limited supervision and may involve medical and social service activities and correspondence. This position requires initiative, creative problem resolution, accurate data entry skills, excellent phone and electronic communication skills, service excellence and a desire to work within a practice that centers around and with a geriatric population. In this role, you will:
  • Accurately enter all clinical patient information into the electronic medical record.
  • Use resources available to the practice for appropriate patient sharing platforms to obtain or search for any missing patient diagnosis, surgical, past medical family social history, and/or allergy information.
  • Contact patients, families, or facilities to obtain any missing patient history information.
  • Obtain all durable power of attorney, guardianship, do no resuscitate, all required consent forms, and other pertinent forms related to legal decisions or decision makers.
  • Attach all pertinent forms to patient chart accurately following standard operation procedures for documentation labeling.
  • Complete workflow in a timely fashion related to obtaining outstanding patients documents, forms, or offering patient portal to patients or authorized representatives.
  • Alerts schedulers to Hospital Discharge for follow up with necessary TCM visits.
  • Provides patients or families with provider biographies and answers any questions regarding the patient's first visit with a Physician Services provider.
  • On-boards all new patients or families to ensure the patient and/or family understand any additional requirements or needed forms.
  • Communicating with patients and families, including answering program telephone.
 
Required Qualifications
  • High School Diploma or GED
  • Minimum one year in the medical field, medical office experience, or related position
  • Ability to operate computer equipment, fax, and photocopy machines necessary
  • Must be proficient in Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint
  • Excellent verbal and written communication skills
  • Strong grasp of medical terminology
 
Desired Qualifications
  • Associate degree
  • Experience with geriatric population and/or caregivers
  • Registered Medical Assistant or Certified Nursing Assistant

Hours
  • Office hours: 8:00 AM – 5:00 PM, Monday through Friday
  • Weekend Rotation
