Key Responsibilities
The Licensed Practical/Vocational Nurse (LPN/LVN) is responsible for providing direct patient care under the supervision of a registered nurse. This role involves implementing the established plan of care, administering treatments and medications as prescribed, and supporting patients and families throughout the end-of-life journey. The LPN/LVN collaborates closely with interdisciplinary team members to promote comfort, dignity, and quality of life, and to ensure the patient care outcomes are met in accordance with hospice standards and regulatory guidelines.
In this role you will be responsible for:
- Provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to end-of-life care
- Implements current nursing practice following a comprehensive assessment and the plan of care
- Provide accurate and timely documentation of patient services to reflect the plan of care
- Participates in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, SW, ST, Dietician, or Clinical Supervisor
- Provides appropriate pain/symptom management. Evaluate patient’s response to treatments/medications
- Ensures clinical notes indicate continuing communication and coordination of services with physicians, facility staff, and team members.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Currently licensed as an LPN/LVN in the State you will be working in
- Minimum 1 year of experience as a nurse in a clinical care setting
- Ability to work collaboratively as part of an interdisciplinary team and possess a strong commitment to the hospice philosophy of care
- Excellent verbal and written communication skills
- Strong organizational and time management abilities
- Solid clinical judgement and the ability to follow care plans and physician orders accurately
Desired Qualifications
- Hospice or home health experience preferred
- A strong understanding of the grief process and experience in supporting individuals coping with loss

- Office Hours: 8:00 am – 5:00 pm, Friday - Tuesday
Pay Range
$24 - $26 USD
