Nestled on a nine-acre campus in San Francisco's Excelsior neighborhood, the San Francisco Campus for Jewish Living (SFCJL) is a center of excellence in providing a continuum of care. This includes the Frank Residences, offering luxurious assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.

What You’ll Do as an Admissions Coordinator:
As an Admissions Coordinator at the San Francisco Campus for Jewish Living, you’ll help manage the admissions process to our Jewish Home & Rehab Center. You’ll work closely with hospitals, referral sources, applicants, and our interdisciplinary team to ensure a smooth and safe transition for new residents—supporting our census goals and maintaining high standards of customer service.
- Support census growth by managing admissions flow and throughput targets.
- Enter and maintain accurate admissions data across EMR, referral platforms, email, and phone.
- Serve as the primary contact for Short-Term and Long-Term Care inquiries; provide tours and respond promptly to prospective residents and families.
- Coordinate with hospital Case Managers to support smooth transitions to skilled nursing care.
- Review and track LTC applications for completeness and appropriateness; follow up with applicants.
- Prepare rooms for incoming admissions and order necessary durable medical equipment (DME).
- Collaborate with Clinical Liaisons to identify care needs and communicate with the interdisciplinary team.
- Complete all admissions paperwork, create EMR profiles, upload resident photos and documentation.
- Verify insurance benefits, input payer details into the EMR, obtain authorizations as needed.
- Maintain relationships with referral sources to grow visibility and increase referrals.
- Represent the organization professionally in both internal and external settings.
- Provide backup support for department colleagues and assist with special projects as needed.

Requirements:
- High school diploma or equivalent required; AA/AS/BA/BS preferred
- Skilled nursing billing and insurance experience preferred
- Proficiency in MS Office (Word, Outlook, Excel) required
- Experience in a fast-paced setting with ability to multitask under pressure
- Excellent communication, customer service, and interpersonal skills
- Ability to build strong relationships with referral sources, applicants, and families
- Must handle sensitive and confidential information with professionalism
