  • Part-Time: 16 hrs/week 
  • Shifts: Friday and Saturday NOC Shift, 11 PM - 7:30 AM
  • Pay Rate $42 - $45/hr for LVN
  • Pay Rate $Starting at $59/hr

This includes the Frank Residences, offering luxurious assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.

What You’ll Do as a Wellness Nurse Supervisor:
As a Wellness Nurse Supervisor, you will support the Health & Wellness Department by supervising caregiving staff, coordinating resident medical services, overseeing documentation and compliance, and facilitating communication with residents, families, and external providers. You will ensure a high standard of care through leadership, training, and hands-on clinical support.

Essential Functions

    • Oversee the reporting process/structure for Medication Technicians & Resident Service Partners– ensuring accurate updates/reporting for shift hand-off.
    • Supervise and manage documentation – ensuring accurate, complete and confidential documentation as it relates to resident medication, treatment, and care records, according to community policies and procedures.
    • Assist Director of Health & Wellness with receiving and processing resident admissions, readmits and discharges.
    • Initiates and leads the implementation of COVID-19 related policies and procedures including visitation.
    • Compile new resident medical charts, thin charts into overflow files and dismantle discharged resident medical charts.
    • Responsible for maintaining all resident medical paperwork in appropriate chart or other designated location.
    • Oversee appointments and transportation for resident medical services.
    • Triage residents if they become severely ill, injured, or present with a change of condition.
    • Manage concerns, issues or questions presented by the resident or resident’s family.
    • Report and document immediately any employee incident/injury.
    • Lead communication with physicians, families, and other appropriate parties, concerning resident status, changes in status, need for appointment, and other health services needs.
    • Assist Director of Health & Wellness with staff hiring and training as directed by supervisor.
    • Oversee the inventory, ordering and stocking or medical supplies and other supplies for operations.
    • Lead and conduct weekly, monthly, quarterly audits.
    • Manage and create team schedules including taking call offs, finding replacements and covering shifts.
    • Coordinate and train new hires for the department.
    • Responsible for developing and leading ongoing educational trainings and in-services for a wide-range of clinical care topics to ensure best practices are followed and consistently maintained by the Med-Techs and Resident Services Providers.
    • Responsible for mentoring new Wellness Nurse Supervisors to ensure successful orientation and onboarding to the Health & Wellness Department.
    • Carry out other duties as assigned by supervisor, cross trained for caregiving and medication responsibilities.
    • Assist in resolving employee relation issues and disputes by working with supervisor and HR.
    • Evaluate staff performance and provide feedback as necessary.

     

    Medication-Related Duties (when applicable)

    1. Administer medications and treatments as assigned within the scope of practice, following the five “R”s: right resident, right time, right medication, right dose, and right route.
    2. Monitor self-medication program of residents when ordered by the physician or assigned by the nurse.
    3. Follow infection control and safety procedures to protect resident and employee from exposure risk.
    4. Pass medications, staying with each resident until the medication is consumed. Maintain upkeep and cleanliness of med carts and medication room.
    5. Responsible for recording and/or restocking of medications from pharmacy after receiving proper instructions. Responsible for counting, locking, and storing narcotics.
    6. Responsible for ordering new, routine, and cycle fill medications as ordered by physician.

     

    Caregiving-Related Duties (when applicable)

    1. Triage and fill out Incident Report for resident and or staff when an incident occurs.
    2. Become familiar with and adhere to each resident’s individual service plan and assist in all care as needed along with completing care plan updates.
    3. Assist with resident activities, encourage, remind, and assist residents to participate in activities.
    4. Assist with cleaning (or disinfecting) resident assistive appliances and/or equipment, as needed.
    5. Dispose of any biohazard waste according to OSHA regulations and community policies.
    6. Carry out all resident care duties following universal precautions with appropriate application and removal of personal protective equipment.
    7. Practice proper body mechanics when lifting and follow safety guidelines according to community policies and procedures.
    8. Answer and respond immediately to resident calls for assistance.

     

 

Requirements:

  • Current and active State of California LVN or RN License.
  • Two years recent LVN nursing experience required and/or two years assisted living experience as a med-tech or caregiver at Frank Residences or comparable care community.
  • New RN graduates might be considered
  • Previous Supervisory/Management experience strongly preferred
  • Assisted Living experience preferred
  • Minimum high school graduate or GED. Preferred coursework in medical terminology, pharmacology, or basic health services.
  • CPR/first aid certification required.
  • Genuine concern for and ability to work with the elderly.
  • Knowledge of the requirements for providing care and supervision to the elderly.
  • Ability to delegate responsibility while maintaining focus of daily activities.
  • Ability to manage and prioritize a complex medication and treatment administration process, within a specified timeframe.
  • Must be able to utilize standard precaution knowledge and infection control measures where required.
  • Organized and knowledgeable of basic medical record filing styles.
  • Possess sufficient communication and language (orally and written) skills to perform job duties, interpret medical information and communicate with residents, other staff, family members, etc., as needed.
  • Physical Demands
  • Ability to lift, push, pull a minimum of fifty (50) pound All lifts and carry greater than 50 lbs. are required to be performed with a two person lift and/or mechanical lift equipment.
  • Ability to stand for extended periods of time
  • Must be able to assist in evacuation of residents during emergency situation
  • Ability to bend, stoop, stretch, twist, sit, and reach
  • Fine motor skills
  • Visual acuity
  • Perform tasks listed above with or without the aid of mechanical devices
