The Role:

As a Wellness Coordinator, you’ll be the cornerstone of our patient care, ensuring that every senior receives the personalized and comprehensive care they deserve. You’ll work alongside healthcare providers to optimize care plans, improve patient outcomes, and enhance overall quality of life. Your role will be vital in creating and maintaining strong relationships with patients, caregivers, providers, and the care team. You’ll help patients manage their conditions, provide support, and offer guidance as they navigate their health journey.

Key Responsibilities:

  • Be the face of Senior Doc at our partner senior living communities, providing expert care and support to residents, staff, families, and healthcare providers.
  • Take vitals and weights on community residents monthly to ensure their health is monitored.
  • Coordinate and communicate care services and health needs for residents.
  • Facilitate telehealth appointments and provide technology support to residents.
  • Conduct wellness checks for patients enrolled in our programs and offer ongoing support.
  • Be a companion to residents, offering emotional support and a caring presence.
  • Participate in community and company events, bringing people together to celebrate health and wellness.
  • Educate and support families on how to manage conditions, teach self-care techniques, and provide resources for better health.
  • Work closely with healthcare providers to ensure smooth, efficient coordination of care.
  • Assist in quality improvement initiatives to enhance the quality of life for our patients.
  • Train and mentor staff members, sharing your knowledge and experience.

Requirements:

  • Medical Assistant Degree required—we’re looking for someone with the skills and experience to excel in patient care.
  • High school diploma, GED, or equivalent.
  • Experience with geriatrics—must have experience working with the senior population.
  • Experience or education in patient care and/or case management.
  • A professional, outgoing personality with a strong sense of initiative.
  • Ability to work independently and thrive in a dynamic environment.
  • Willingness to travel daily to assigned locations and attend community events outside the typical Monday-Friday, 8-5 schedule.
  • Ability to stand for extended periods.
  • Phlebotomist License (preferred but not required).
