Licensed Clinical Social Worker
Summary:
The Licensed Clinical Social Worker provides clinical social work services to low/high-risk patients/caregivers coping with the diagnosis of a life-threatening or chronic illness. Social Work services to be provided include psychosocial assessment, counseling, information/referral/case management, crisis coverage, facilitation of patient and family groups, and grief care.

Knowledge/Skills/Abilities:
- Excellent written and verbal communication skills, with ability to communicate and collaborate with all levels of employees, physicians, patients, and families.
- Strong computer and organizational skills.
- Ability to prioritize assigned tasks and the needs of the patients.
- Ability to work autonomously while completing assigned tasks and maintaining workflow needs.
Key Job Responsibilities:
- Initiates contact with patients referred for psychosocial issues.
- Conducts psychosocial assessments of patients and families and develops plans to address needs.
- Provides direct interventions to patients and their family/caregivers including individual, family and/or group counseling and crisis intervention as needed.
- Identifies patient/caregiver strengths, coping skills and support resources.
- Facilitates connection to relevant community, governmental and/or national resources for patient and family/caregiver.
- Works in collaboration with physicians and other healthcare personnel in patient evaluation and treatment, to further their understanding of significant social and emotional factors underlying a patient’s health problems.
- Assists patient with adjustment to treatment, including but not limited to problems of depression, anxiety, denial, anger and noncompliance with medical regimen.
- Provides crisis intervention when appropriate.
- Promotes professional growth and development of oncology Social Work.
- Provides supervision to Social Work interns.
- Documents service provision, consultation and correspondence in the patient’s medical record.
- Respects the dignity and confidentiality of patients.
- Maintains a good attendance record.
- Maintains a professional attitude and appearance.
- Possesses good verbal and written communication skills that lend to a team-oriented work environment.
- Performs other duties as assigned.
Supervision Provided by this Position:
- This position will supervise the Social Work Interns.
Physical Demands/Conditions:
- Exposure to patient body fluids.
- Ability to react quickly to emergency situations.
- Ability to read and write to communicate both orally and in writing with other individuals.
- Ability to lift up to 25 lbs.
- Ability to stand, reach, bend, walk, twist and sit in one place throughout shift.
- Ability to use hands and feet together for repetitive, coordinated motions.
- Ability to use hands for gripping and squeezing as well as repetitive hand and wrist motion.
- Good hand-eye coordination.
