Clinical Support Specialist

The CareBridge Clinical Support Specialist supports and collaborates with patients, medical professionals, and vendors to ensure comprehensive care for patients with physical, developmental, and intellectual disabilities. This is a frontline role which partners with an interdisciplinary care team to provide care coordination to patients and thus improve access to critical services. Clinical Support Specialists will assist patients in scheduling visits with CareBridge interdisciplinary team members, facilitate care coordination with external medical providers/ specialists, and procure supplies and equipment in a timely manner to support independence in the home.

Responsibilities:

  • Arrange for and help coordinate prescribed laboratory tests and/ or diagnostic procedures.
  • Support order, delivery, and receipt of DME/ medical supplies as identified by patient care team
  • Identify and schedule referrals with external providers, including specialty and home health care
  • Provide day-to-day support to care team (including RNs, NPs, Pharmacists, Social Workers, etc.)
  • Coordinate visits with other CareBridge interdisciplinary care team members, as necessary
  • Orders diagnostics, labs, referrals, home health and all equipment and supplies accurately and timely and ensures receipt.
  • Has advanced knowledge of Medicare and state Medicaid coverages of DME and supplies
  • Maintain excellent punctuality and attendance during work hours and adhere to schedule
  • Manage large amounts of inbound and outbound inquires and document all correspondences within systems timely and accurately, all while targeting for a first call resolution
  • Other duties as assigned.

Qualifications:

  • Passion for helping patients with physical, developmental and intellectual disabilities
  • A warm, engaging personality with the ability to connect with individuals virtually via phone and/or video
  • 3+ years’ experience in a healthcare environment, medical back office experience preferred
  • Direct experience working with DME greatly preferred
  • Has advanced knowledge of Medicare and state Medicaid coverages of DME and supplies
  • Certified Nursing Assistant, Medical Assistant or Licensed Practical Nurse a plus
  • Experience delivering persuasive messaging and empathy.
  • Expertise in phone etiquette and professional phone demeanor
  • Solution-oriented and a creative problem solver
  • Inspires a positive and collaborative working environment.
  • Ability to work in a metric-driven and goal-oriented environment.
  • Experience with conflict resolution, defusing, and de-escalating calls
  • Calm, unhurried approach that inspires confidence.
  • Highly organized and detail-oriented, able to multi-task
  • Self-starter with a propensity to follow-up and follow-through
  • Experience with Thrio and Elation systems preferred.
