Patient Intake and Authorization Coordinator

Position Overview:

The Patient Intake and Authorization Coordinator position is responsible for managing the intake and start of care for patients who are referred to Centria. This includes obtaining and maintaining the authorizations and all required clinical documentation for the patient's records, completing quality analysis, monitoring clinical documents, and conducting benefits and eligibility checks on all patients. 

Success measures include adherence to Centria policies, flawless execution of tasks, and meeting or exceeding personal qualitative and quantitative key performance indicator (KPI) metrics.

 
Responsibilities:

  • Manage the intake and start of care for referrals.
    • Set up an electronic medical record profile.
    • Obtain all required clinical documentation.
    • Obtain authorization/script.
    • Coordinate with the Case Manager, Insurance, and Client Service Manager until services have started.
  • Ensure all necessary documentation is uploaded to the patient's profile.
  • Upload a copy of the patient's insurance card (front and back) into the patient's profile in an electronic medical record.
  • Ensure scripts are uploaded to the patient profile.
  • Download any nursing documentation that is received and split into the patient's files.
  • Conduct benefits and eligibility checks on all patients.
  • Obtain any authorizations needed for all patients and update authorization in patient's profile.
  • Work with clinical to maintain updated authorizations for all clients. Submitting to appropriate insurance for all requests including, but not limited to, initial, increase/decrease, renewal, etc. Update new start date on the MSA form.
  • Manage medical record requests and obtain updated scripts on an ongoing basis.
  • Pull all authorizations from portal to upload to client's profile. Send email to appropriate parties with updated authorization.
  • Complete monthly Intake Assessment.
  • Send a discharge letter to appropriate parties when a patient is discharged.
  • Update patient's status to discharged/hold when notified that a patient's status has changed.
  • Complete ongoing quality analysis to review patient charts and provide direction/education as required.
  • Call physicians/hospitals/case managers/other stakeholders as needed for maintenance of clinical records.

Managerial Responsibility

This role acts as an individual contributor with no direct reports.

Required Qualifications, Skills, and Knowledge:

Education/Licenses

  • College degree or equivalent, minimum requirement
  • Licensed Practical Nurse (LPN) preferred

Work Experience

  • Working knowledge of Private Duty Nursing and Attendant Care
  • Knowledge in obtaining authorizations

Equipment and Technology Requirements

  • Basic computer skills
  • Working knowledge of laptop/desktop PC
  • Proficiency in Microsoft Suite
  • Proficiency in G Suite

An initial assessment of competency will be completed during the interview period and evaluated on an ongoing basis.

  • Proficient in Microsoft Office Suite, especially spreadsheet programs;

  • Ability to apply and adopt practices and techniques to the specifications set by senior leadership;
  • Effective communication skills, both written and verbal, with internal and external stakeholders;
  • Ability to build rapport with team members, explain complicated information in an approachable and easily understood manner, effectively handle challenging phone conversations, and represent the Centria brand;
  • Ability to organize, prioritize and handle multiple tasks, adhere to established deadlines, and produce work that consistently meets or exceeds team benchmarks;
  • Results-driven with the aptitude to apply critical thinking skills and problem-solving;
  • Proven adaptability with a willingness to work both collaboratively and individually to achieve desired business outcomes;
  • Excellent interpersonal, customer relationship, and listening skills;
  • Demonstrated strong work ethic with attention to detail, accuracy, and quality;
  • Established track record of generating error-free work.

Pay Range:  $48,000 - $55,000

Working Conditions:

Centria’s office hours are Monday through Friday from 8:30am-6:00pm. Additional time may be required to complete above work or meet company objectives.

Physical Requirements:

While performing the duties of this job, physical requirements such as bending, reaching, lifting, pushing, or pulling up to 30 pounds may be required. This role will require sitting most of the day as well as walking and standing periodically. This role may require close visual acuity on computer screens or monitors and the ability to analyze data and figures on a screen.
