Care Navigator Supervisor

The Opportunity
Join us in tackling healthcare for rural America! We’re seeking a Michigan Field Care Navigator Supervisor to manage a team of Homeward Care Navigators. A Homeward Care Navigator works with our members and their providers to achieve their best health. The Homeward Care Navigator conducts proactive telephonic, video, in-home, and in-person outreach to build relationships with members and connect them with services they need- medical, behavioral, and social- and address gaps in their care. As the Field Care Navigator Supervisor, you will report to the Navigation Service Line Leader and will be responsible for hiring, training, coaching, mentoring, and performance monitoring a Field Care Navigator team. You will be Homeward’s Care Navigation subject matter expert.
What You'll Do
- Manage the day-to-day operations for a team of Michigan Field Care Navigators by providing direct leadership, support, and guidance.
- Take ownership of supervisory decisions by confidently addressing routine operational issues and escalating thoughtfully with well-prepared recommendations when needed.
- Assume primary responsibility for hiring, training, development, and performance monitoring of Care Navigators in collaboration with the Navigation Service Line Leader
- Become a subject matter expert in care navigation for Homeward
- Conduct regular team meetings, one-on-one sessions, and performance evaluations to assess individual and team performance.
- Foster a positive and inclusive work environment that promotes teamwork, professional growth, and employee engagement.
- Conduct regular quality assurance reviews via direct observation of virtual member visits and management of member feedback.
- Conduct regular audits of member records, documentation, and care plans to ensure compliance with regulatory standards and company guidelines.
- Identify opportunities for quality improvement and implement corrective actions as needed to enhance the quality and effectiveness of member care services.
- Serve as a point of contact for the Michigan Field Care Navigators for concerns, grievances, and conflict resolution, providing guidance, mediation, and support as needed in a timely manner.
- Collaborate with Navigation Service Line Leader and cross-functional teams to streamline processes, enhance workflows, and drive operational efficiency.
- Consistently prioritize and complete time-sensitive tasks on schedule, demonstrating strong organizational and time management skills.
- Foster open and proactive communication with colleagues and leaders by sharing timely updates on key responsibilities and team priorities.
What You Bring
- Registered Nurse degree from an accredited school of nursing is preferred but not required
- Licensed/Certified/eligible to practice in the United States
- Licensure in Michigan (within 3 months) and ability to become licensed in additional states
- Strong decision-making skills with ability to act independently and confidently within defined role scope.
- Proven ability to manage multiple priorities with minimal follow-up, demonstrating excellent organizational and time management skills.
- Three years of supervisory or leadership experience in a healthcare environment
- Two years of supervisory experience in a Hybrid Model or Full Remote Model is preferred.
- Passion for delivering care in rural America
- Experience working on a small, fast-paced team is a plus
- Experience working with connected devices/loT is a plus
- Experience with various EHR platforms (Epic, Cerner, Athena, etc.) is required
- Strong organizational skills and ability to manage multiple priorities in a fast-paced environment
- Commitment to quality, safety, and continuous improvement in patient care delivery.
- Experience working in a start-up environment preferred
- Experience working in value based healthcare is a plus

The base salary range for this position is $70,000 - $85,000 annually. Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, location, competencies, and experience. Base pay is one part of the Total Package provided to compensate and recognize employees for their work at Homeward Health. This role is eligible for an annual bonus, stock options, and a comprehensive benefits package.
