Pediatric Nurse Practitioner (In-Home Care and Virtual)

We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. The primary location for this role is Washington DC, travel to patient's home, and the expected schedule is Mon-Fri 10:00am - 06:00pm local time.  Active Washington DC license is required. 

 

What You’ll Do  

As a full-time Pediatric Nurse Practitioner (NP) & Home Care Specialist, you’ll serve as a direct clinical provider within an innovative care model that blends virtual acute care and hands-on, in-home interventions. You’ll support medically complex pediatric patients and their families during high-acuity moments—delivering compassionate, high-quality care at their doorsteps—helping to reduce unnecessary hospital visits. You will: 

  • Practice within a transformative pediatric care model designed for medically complex children and their caregivers
  • Provide in-home and virtual healthcare services—spanning urgent, routine, and consultative support
  • Triage pediatric needs and deliver tailored virtual and in-home interventions
  • Lead in-home clinical evaluations including history, focused exams, diagnostics, and treatment initiation
  • Collaborate with an interdisciplinary team—physicians, nurses, pharmacists, therapists, social workers, dieticians, and navigators
  • Support member onboarding, post-discharge care planning, and home visit coordination

What You Bring & How You Qualify  

First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. You will need: 

  • Current residence in Washington, DC
  • Active Nurse Practitioner license in Washington DC required, Certified Pediatric Nurse Practitioner preferred
  • Willingness to obtain licensure in other Imagine Pediatrics markets in response to evolving business needs (support provided)
  • Active DEA registration or eligibility
  • 3+ years’ experience in high-volume and acuity pediatric acute care (Pediatric Urgent Care or Pediatric Emergency Department)
  • BLS, ACLS, and PALS certifications required
  • Proven success managing medically complex pediatric patient
  • Telehealth experience is strongly preferred
  • Prior professional experience with in-home care services is preferred
  • Experience leading interdisciplinary teams and guiding remote clinical assessments
  • Current credentialing and CME compliance
  • Comfort working in a tech-forward care environment
  • Active and valid driver’s license, insurance, and clean driving record
  • Passion for addressing healthcare inequality and expanding access
  • Reliable internet connection and a private, HIPAA-compliant workspace
  • Bilingual in English and Spanish preferred 
  • Perform other duties as assigned. 

 

What We Offer (Benefits + Perks)

The compensation range for this role is $100,000 - $145,000 and includes a $5,000 annual bonus. In addition to competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable). When determining compensation, we analyze and carefully consider several factors including job-related knowledge, skills and experience. These considerations may cause your compensation to vary.
