URGENT NEED! 3-6 month contract with possibility for extension

 

Schedule: Full-time (40 hours/week), Monday-Friday

Compensation: $80/hour (Hourly, Contract)

Work Location: Hybrid - Remote and Community-Based (Greater Boston)

Position Summary:

Legacy Care Group is currently looking for a qualified Clinical Assessment and Care Planning Manager to work for Commonwealth Care Alliance (CCA). The Manager, Clinical Assessment and Care Planning provides leadership for and manages the assessment and care planning functions of the Unit. This position is responsible and accountable for all aspects of the Clinical Assessment operations, including ensuring that the care planning team is fully scheduled and engaged in ensuring the timely comprehensive clinical assessments for all assigned members. The manager is accountable for ensuring that assessments are comprehensive and of high quality and include medical, behavioral health, and social needs, long term services and supports (LTSS) as covered in the MDS-HC and Supplemental tools.

The Manager, Clinical Assessment and Care Planning ensures the following: all assessment nurses are fully scheduled, all reachable members are assessed within specified time frame; reassessment MDSs are completed on time and to specified requirements with consistently high quality; members are assessed, and the member data is accurately entered and handed off to CCA in a timely manner.


Supervisory Responsibilities

  • Directly supervises RN Nurse Assessors
  • Provides clinical backup by conducting urgent assessments when needed
  • Oversees training and competency development for new clinical staff
  • Supports performance monitoring and quality improvement

Essential Duties & Responsibilities

  • Oversees all aspects of the operational and clinical work of the RN Nurse Assessors, including comprehensive MDS-HC and Supplemental Assessments
  • Oversees quality of clinical work by assessment nurses
  • Delivers training when necessary
  • Tracks assessment completion rates and engages in continuous improvement process to meet productivity and quality goals as needed
  • Individual will be responsible for making high-level clinical, programmatic, and operational management decisions
  • Support teams and track Key Performance Indicators (KPI) related to clinical care for members

  • Ensure confidentiality of member and company proprietary information is maintained

Qualifications

Education & Licensing:

  • Bachelor's Degree in Nursing with current nurse licensure in good standing in Massachusetts
  • CPR certification required

Experience:

  • 5+ years demonstrated clinical competence in assessing patients and care plan development, including experience with MDS-HC
  • 3+ years supervisory experience

Knowledge, Skills & Abilities

  • Proficient with Microsoft Office suite; experience documenting in electronic health records and ability to learn and use new systems
  • Proficient with Excel
  • Excellent communication skills
  • Exceptional knowledge of the array of medical, behavioral health, social and long-term services and supports offerings available across Massachusetts, and the extent to which each can be leveraged to support individuals with complex needs.
  • Proven skills, knowledge base and judgment necessary for independent clinical decision-making
  • Excellent organizational, time-management and problem-solving skills
  • Ability to function effectively as part of a multi-disciplinary team
  • Effective oral and written skills
  • Strong interpersonal skills
  • Commitment to social justice in medicine and an understanding of healthcare reform
  • Effective teaching and mentorship skills
  • Able to support teams with utilization of telehealth technologies (video, chat, etc.), when appropriate, as an approved modality for a variety of clinical care and care management services.

Language: English. Bilingual is a plus!


Working Conditions

  • Hybrid work model: Combination of remote and home/community-based visits
  • May require travel within service areas to conduct or supervise in-home assessments

Physical Requirements

Ability to function in a busy clinical environment and to make home visits in a community-based setting. Visual and hearing ability sufficient to comprehend written/verbal communication and perform physical exams and assessments of patients.
