Self-ID Questions

We recognize that people’s identities are complex. The Equal Opportunity Employment Office options do not usually capture how people identify.  To work toward a better solution, we are providing these Self-ID options in the Voluntary Demographics section below. You may choose: "I prefer not to answer" any or all of the self-ID questions.

At WellPower we embrace diversity, equity, inclusion, & belonging and your self-ID will only be used for operational purposes, in aggregate form and has no impact on your application.

We know that these lists are still incomplete and we encourage you to use the self-describe boxes if you want to identify in a way that is not represented on these lists.
