As a Home Health Registered Nurse, you'll provide comprehensive skilled nursing care to patients in their homes, helping them recover, manage chronic conditions, and achieve their health goals in the comfort of their own environment.  This role is perfect for an RN who values independence, clinical decision-making, and building meaningful relationships with patients and families. You'll manage your own caseload from admission to discharge, collaborating with physicians and a multidisciplinary team while having the autonomy to make clinical judgments that directly shape patient outcomes.

  • Deliver skilled nursing services requiring specialized expertise according to physician orders
  • Initiate diagnostic, preventive, and rehabilitative nursing procedures
  • Demonstrate proficiency in phlebotomy, medication administration, and wound care
  • Monitor and respond to changes in patient conditions promptly

Manage Your Caseload

  • Assume primary responsibility for assessing, planning, coordinating, implementing, and evaluating care
  • Develop, implement, and revise individualized plans of care based on patient needs
  • Make clinical decisions about when to involve other disciplines or escalate concerns
  • Plan and coordinate patient discharges from services
  • Maintain accurate documentation within 24 hours of visits

Teach and Empower Patients

  • Educate patients and families on home nursing procedures and self-care techniques
  • Teach disease prevention, injury management, and health promotion strategies
  • Work toward patient and caregiver independence as the ultimate goal
  • Provide counseling appropriate to each patient's condition and learning needs
  • Use evidence-based teaching materials and techniques

Coordinate Multidisciplinary Care

  • Collaborate with physicians, therapists, social workers, and other healthcare professionals
  • Initiate and participate in client care conferences for complex cases
  • Assign and supervise home health aide services, conducting required supervisory visits
  • Supervise LPN/LVNs and paraprofessionals providing care
  • Ensure continuity of care across all providers

Maintain Clinical Excellence

  • Communicate significant findings, problems, and changes to physicians and supervisors
  • Report unsafe conditions promptly to appropriate parties
  • Participate in quality improvement initiatives and in-service programs
  • Stay current with best practices and regulatory requirements
  • Follow infection control, safety, and documentation policies

  • Proficiency in OASIS documentation and care planning
  • Excellent time management and organizational abilities
  • Effective communication and teaching skills
  • Ability to work independently with minimal supervision
  • Comfort working in diverse home and facility environments
  • Strong clinical judgment and decision-making capabilities
  • Understanding of Medicare, Medicaid, and third-party payor requirements

What Makes You Successful

You're a confident, experienced RN who thrives on independence and variety. You enjoy the challenge of managing complex cases and making autonomous clinical decisions while knowing when to collaborate with your team.  You're an excellent teacher who can break down complex medical information into understandable terms for patients and families. You're patient, adaptable, and comfortable working in varied home environments, from well-appointed houses to more challenging settings.  You're organized and self-directed, able to manage your schedule, prioritize competing demands, and handle the daily changes that come with community-based nursing. You understand that home health nursing isn't just about clinical skills - it's about building trust, respecting patients' autonomy in their own homes, and working toward independence rather than dependence.

Why Join Us?

Home health nursing offers the perfect balance of autonomy and support. You'll have the freedom to manage your own caseload and make clinical decisions while being backed by experienced clinical managers and a collaborative team.  You'll experience the satisfaction of seeing your patients progress from admission to discharge, knowing that your teaching and interventions helped them achieve independence. We provide comprehensive orientation, ongoing clinical education, and opportunities for professional growth including OASIS certification and specialty training.

Work Environment and Schedule

This position requires daily travel to patient homes and occasional facility visits within your service area. You'll work independently in varied home environments, adapting to different settings and patient situations throughout your day.  Schedule changes are common based on patient needs, new admissions, and staffing requirements. Flexibility is essential, and on-call responsibilities may be required. Physical demands include standing, walking, bending, and lifting up to 50 lbs with assistance.

If you believe you belong to any of the categories of protected veterans listed below, please indicate by making the appropriate selection. As a government contractor subject to the Vietnam Era Veterans Readjustment Assistance Act (VEVRAA), we request this information in order to measure the effectiveness of the outreach and positive recruitment efforts we undertake pursuant to VEVRAA. Classification of protected categories is as follows:

A "disabled veteran" is one of the following: a veteran of the U.S. military, ground, naval or air service who is entitled to compensation (or who but for the receipt of military retired pay would be entitled to compensation) under laws administered by the Secretary of Veterans Affairs; or a person who was discharged or released from active duty because of a service-connected disability.

Form CC-305
OMB Control Number 1250-0005
Expires 04/30/2026

Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
