Why This Role Matters
The Patient Onboarding Specialist is responsible for accurately and efficiently entering and managing all new patient information following the initial demographic intake completed by the intake team. This role ensures that all patient records are complete, accurate, and properly documented within the system.
What You’ll Do
- Accurately enter and maintain comprehensive patient data, including past medical, family, and social history, pharmacy information, durable power of attorney (DPOA) details and documentation, medication lists, and allergy information.
- Proactively identify and obtain any missing documentation, including outstanding consents and required new patient paperwork, to ensure a complete and compliant onboarding process.
- Receive, review, and convert patient and physician referrals in accordance with agency policies
- Accurately enter, track, and maintain referral and patient data in company systems
- Serve as a key point of communication between patients, referral sources, clinical teams, sales, and leadership
- Answer inquiries about agency services and provide appropriate community resource referrals
- Coordinate referrals with supervisors and schedulers to support timely patient evaluations
- Maintain organized systems to ensure continuity and quality of patient care
- Provide excellent internal and external customer service, including greeting visitors professionally
- Prepare referral tracking and trending reports as needed
- Credential new physicians and facilities following established procedures
- Identify workflow gaps and recommend improvements to enhance efficiency
- Ensure compliance with all federal, state, and local regulations
Required Qualifications
- High school diploma or GED
- Interpersonal skills and ability to communicate professionally and effectively
- Knowledge of medical terminology
- Strong verbal and written communication skills
- Ability to operate computer and office equipment
- Proficiency in Microsoft Office Suite and data entry skills
- Responds to suggestion constructively
- Skilled in conflict resolution
- Proven interpersonal, coordination, and leadership skills
- Reliable transportation with valid automobile insurance
Preferred Qualifications
- Associate's degree preferred
- Minimum 1 year of experience in a healthcare setting preferred
- Knowledge of Medicare, Medicaid, and third-party payor guidelines preferred
Location
- Office Location: Jackson, MI
Schedule
- Monday-Friday, 8:00 AM – 5:00 PM (on- site)
- Remote weekend rotation with one day off before and after the rotation
Pay Range
$19 - $22 USD
