What You’ll Do
Reporting to the Chief Clinical Officer, this role is responsible for driving execution of Strive’s Total Cost of Care (TCOC) strategy—translating enterprise strategy into operational results. Supporting the CCO and Nephrology and Cardiology Medical Directors, this leader partners closely with clinical leadership, market teams, and central operators to identify, prioritize, and manage initiatives that reduce unnecessary medical expense, improve quality outcomes, and accelerate value-based care success.
The Day to Day
- Identify top TCOC opportunities using claims, utilization, and care management data
- Partner with the Chief Clinical Officer, Nephrology and Cardiology Medical Directors, and the Central TCOC Strategy & Analytics team to develop and drive cross-functional action plans and execution of TCOC priorities (e.g., CHF, ESRD, Optimal Start Dialysis Transitions)
- Monitor TCOC performance metrics and initiative tracking; Lead accountability discussions (e.g. Monthly TCOC Reviews)
- Translate clinical and financial opportunities into operational initiatives
- Ensure adoption of enterprise TCOC tools, workflows, and playbooks
- Serve as thought partner to clinical leaders on medical expense strategy and execution
- Contribute to enterprise-wide TCOC learning agenda, best practice sharing, and innovation pipeline
- Meet in person with internal and/or external stakeholders to facilitate team and business priorities/opportunities. Business travel may be required for opportunities to connect with stakeholders, serve patients, and attend Strive-sponsored team events.
Minimum Qualifications
- Bachelor’s Degree in a relevant field
- 9+ years in value-based care or population health environments (payer or provider)
- 5+ years leading TCOC, care transformation, or medical expense reduction programs
- Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms.
- Ability to travel and be onsite to meet business needs.
Preferred Qualifications
- Demonstrated experience leading cross-functional teams and initiatives
- Strong analytical acumen—ability to interpret claims/utilization data and guide decision-making
- Fluency in project management, initiative planning, and performance tracking with operational follow-through
- Financial literacy—understanding of medical cost drivers and VBC incentive structures

- Polished communication and presentation skills; ability to influence and drive alignment across clinical and operational stakeholders
- Strong problem-solving skills with the ability to thrive in a fast-paced, dynamic environment.
Annual Base Salary Range: $153,000 - $191,000
