Specialty Clinical Pharmacist - Chronic Care Management

We are seeking a Specialty Clinical Pharmacist to work within the pharmacy and clinics at Redeemer Health to support the use of specialty medications. The Specialty Clinical Pharmacist works in collaboration with providers, clinic staff, and patients to facilitate safe, effective, and affordable treatment to improve clinical and quality of life outcomes. You will provides therapeutic recommendations based on clinical guidelines, patient-specific factors, and managed care requirements. You will counsel patients on the appropriate use and storage of medications, adherence strategies, side effect management, and other strategies to improve outcomes. When therapy is suboptimal or inappropriate, you will performs clinical interventions to improve patient response to therapy. Through collaboration with the Specialty Pharmacy Liaison and other members of the pharmacy team, you will provide exceptional customer service and increase utilization of the health system specialty pharmacy.
Here's what you'll be doing:
- Collaborating with prescribers, clinic staff, and the Specialty Pharmacy Liaison to identify patients utilizing or being prescribed specialty medications.
- Educating patients about specialty pharmacy services and the benefits of using the health system specialty pharmacy with the goal of increasing prescription capture.
- Performing medication reconciliation to identify potential drug-drug or drug-disease state interactions and ensure optimal treatment.
- Providing evidence-based therapeutic recommendations tailored to the unique needs of patients.
- Providing appropriate therapeutic substitution recommendations in response to managed care restrictions.
- Performing comprehensive patient counseling, including disease state; medication administration, storage, and disposal; and non-pharmacologic strategies for disease management.
- Identifying potential issues related to therapy underutilization, overutilization, or inadequate response.
- Monitoring lab work and test results to ensure appropriate therapeutic response.
- Providing recommendations for clinical interventions to improve patient therapeutic and quality of life outcomes.
- Supporting prior authorization, denials, and appeals processes.
- Performing patient outreach to coordinate care, assess treatment adherence and response, ensure progress toward achieving treatment goals, and facilitate ongoing medication adherence.
- Providing exceptional customer service to patients, provider office staff, and external business associates.
What you'll bring to the table:
- BSPharm or PharmD degree from an ACPE-accredited school of pharmacy.
- Licensed Pharmacist in the State of Pennsylvania.
- Pharmacy Controlled Substance License in the State of Pennsylvania
- At least two years' pharmacist experience, preferably in a specialty or oncology setting.
- Ability to collaborate with internal and external customers to promote quality care and customer service.
- Capable of working both independently as well as part of an interdisciplinary team.
- Critical thinking to identify, assess, and respond to medication-related issues.
- Ability to multi-task and organize workload for efficient use of time.
- Ability to work effectively and efficiently under tight deadlines, high volumes, and multiple interruptions.
- Strong communication skills, including active listening, speaking, and effective writing.
You'll stand out if you also have the following:
- BCOP or CSP certification.
- Experience with health system electronic health records (e.g., Epic, Cerner, Athena) and specialty pharmacy management platforms (e.g., Asembia-1, Therigy, Compass Rose).
- Previous experience working with physician offices and/or clinics.
- Knowledge of and experience with health system specialty pharmacy operations.
