Our mission-driven, person-first company is seeking a self-motivated and empathetic Outreach Liaison to join our team. This role is essential to connecting patients with the critical mental health and social services that they need.
The Outreach Liaison role requires strong interpersonal skills, the ability to build trust quickly, and a relentless drive to help patients access the care they deserve. As an Outreach Liaison, you will play a pivotal role in Author Health’s approach to care by meeting in-person with patients with SMI, SUD and dementia - in the community, at their homes, primary care physician offices, and at local hospitals. Your focus will be on building connections and relationships, and helping these patients and their caregivers schedule and attend Author Health health care appointments.
As an Outreach Liaison, you will be responsible for outreach aimed at helping connect patients to the right type of Author Health services, providing an excellent patient experience, helping growing our program, documenting member interactions thoroughly and timely, and providing seamless handoffs to our clinical team. WHAT YOU WILL DO:
- Conduct in-person outreach to doctor’s offices, hospitals and patients’ homes to educate potential patients and caregivers on the mission and value of Author Health, understand their health services needs, help them navigate care options and schedule recommended health care appointments with Author Health
- Independently organize and prioritize your workload, and problem solve to meet monthly outreach and enrollment goals
- Utilize Author Health’s proprietary CRM and electronic medical record to track outreach and document member consent and interaction notes accurately
- Work collaboratively with internal outreach and care teams to ensure a seamless transition into ongoing care at Author Health
- Aid in capturing data and providing feedback on outreach strategies to improve and evolve our approach, ensuring that we remain patient-focused and effective
- Participate in virtual meetings and maintain motivation in a fully remote, dynamic work environment
WHAT WE ARE SEEKING:
- 3+ years of experience in customer service/sales, with cold outreach experience preferred
- 3+ years of experience in healthcare outreach, community engagement, or related fields
- Multi-lingual (Spanish and/or Haitian Creole) candidates preferred
- Strong ability to initiate conversations and build connections quickly, especially with individuals with vulnerable health care needs
- Experience in healthcare, social services, or mental health services, with an understanding of seniors’ needs and challenges
- Proficiency in critical thinking, time management, and multitasking
- Able to work independently and able to meet monthly outreach goals successfully
- Enthusiasm for working in an innovative, fast-paced startup environment
- Excitement at spending majority of time visiting patients in their homes, at the hospital, physician offices, or other in-person locations
- Must have access to a car, and be able to drive up to a 100 mile radius daily with appropriate reimbursement for mileage
- Strong tech proficiency and comfort using multiple software systems for daily work
- Basic proficiency in Google Suite, and video conferencing platforms
- Adherence to HIPAA regulations and confidentiality
