We are looking for a committed and empathetic individual to join our team as a Care Manager. This vital role involves coordinating care for adults with behavioral health needs alongside their medical treatment. The ideal candidate will have a solid background in nursing with a focus on medical and behavioral health and possess outstanding communication and organizational abilities.
WHAT IS YOUR SUPERPOWER?
- Perform comprehensive assessments of patients' behavioral health and medical needs.
- Remove barriers to engaging in health care services to ensure medication and treatment adherence.
- Create personalized care plans in collaboration with patients, their caregivers, and healthcare providers.
- Oversee and coordinate the execution of care plans to ensure they achieve the patients' goals and objectives.
- Educate patients on health conditions, treatment options, and self-management strategies.
- Advocate for patients to ensure they access appropriate medical and behavioral health services in a timely manner.
- Work with interdisciplinary teams, including psychiatrists, nurse practitioners, licensed clinical social workers, and field workers to enhance patient care outcomes.
- Regularly monitor and evaluate patient progress, adjusting care plans as needed.
- Maintain precise and current documentation of patient interactions and interventions.
- Engage in case conferences, team meetings, and quality improvement initiatives.
- Participate in daily huddle meetings.
WHAT WE ARE SEEKING:
- Current Registered Nurse (RN) licensure in Texas, Florida, or any multi-state compact jurisdiction.
- At least 3 years of experience in behavioral health or medical case management.
- Strong understanding of behavioral health conditions, treatments, and evidence-based practices.
- Strong understanding of chronic medical conditions in adults that lead to ED and inpatient hospital presentations, and interventions to prevent those hospital presentations.
- Excellent assessment, interpersonal, and communication skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Proficiency in electronic health records (EHR) and other healthcare technologies.
- Bilingual in Spanish-English or Haitian Creole-English with strong verbal and written communication skills in both languages.
- Dedication to patient-centered care and advocacy.
Preferred Qualifications:
- Certification in Case Management (CCM) or a similar credential.
- Experience working with diverse populations and an understanding of cultural considerations in healthcare.
- Knowledge of community resources and support services for patients with behavioral health and complex medical needs.
- Prior experience in a hospital, outpatient clinic, or community health setting.
