
Lead Intake/Information Coordinator

We are seeking an Intake Coordinator who will be responsible for coordinating new intake referrals, gathering medical records, hospice orders, and patient insurance information, in compliance with By the Bay Health's organizational policies, procedures, applicable laws and regulations.
Schedule:
Hybrid; Tuesday - Saturday; 8:30am - 5:00pm

Larkspur
By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities include (but are not limited to):
- Oversee the coordination of weekday and weekend admissions including review of sign-outs, closing out old charts, and coordinating comfort kit deliveries on weekends
- Ensure adequate daily staffing on Intake team
- Ensure review of all inquiries and referrals for admission to Hospice Care program
- Develop and maintain reference materials and administrative procedures for Intake staff
- Provide training to new Intake Coordinators and oversee training on new processes.
Education/Experience:
- Associate degree (A.A.) or equivalent from a two-year college or technical school, or equivalent combination of education and experience
- Minimum two years’ experience performing clerical duties preferably in a healthcare setting
Pay Range:
$29.00 - $43.75 hourly
