ABOUT THE ROLE

We are looking for a Case Manager Leader eager to build and deliver care in a best-in-class innovative PHP/IOP program! The case management team will have significant responsibility and voice in our PHP & IOP care model, and as a core team member for our first clinic, you will help establish the processes that will lay the groundwork for expansion and will have significant potential for leadership and growth within Overstory.

You will be a critical member of our multidisciplinary care team, focused on coordinating client admission into Overstory’s clinical programs, connecting patients with essential resources, escalating core barriers to patient graduation, and supporting patients and family throughout the program. The Case Manager Leader will work closely with the full team, including our Clinic Director and Medical Director, to ensure patients receive world-class services.

QUALIFICATIONS

  • Lives in Boston, MA or is willing to relocate
  • W2 position, full-time with availability for 40 hours per week (we will try to be as flexible with your schedule given IOP/PHP program and staffing requirements)
  • Graduated from an accredited college or university with a minimum of a Bachelors (BSW) or Master’s Degree in Social Work (MSW), counseling, or health-related field
  • Active licensure (e.g., LMHC, LCSW, LICSW) in social work or a related field is preferred
  • At least (5) years of prior experience in case management, particularly in a behavioral health setting
  • A minimum (2) years of experience leading or supervising a team of case manager, social workers, and/or care navigators
  • Strong experience leveraging local community and social services in Boston, MA to connect individuals with essential resources
  • Confidence working with Medicaid populations and clients with complex social needs
  • Proficiency in Microsoft Office applications and ability to learn department and job-specific software systems (e.g., Slack, Zoom, Electronic Health Record, etc.)

RESPONSIBILITIES

  • Admissions Coordination: Engage prospective clients (both inbound and outbound) to educate them about Overstory and facilitate their admission into the program (e.g., document client information, schedule an initial intake appointment, verify benefits, etc.)
  • Case Management: Work with health plans, pharmacy, and referring physicians as needed to ensure that all information, authorizations, and approvals are received as required
  • Care Coordination: Collaborate with the full team to develop and implement personalized treatment plans that focus on both behavioral & social needs
  • Resource Navigation: Connect patients with community resources such as food, employment, and financial support to prevent recurrent escalating crises
  • Support & Education: Provide patients and their families with information about their diagnosis, treatment offered at Overstory, and guidance on next steps after graduating
  • Documentation: Maintain accurate and detailed patient records, ensuring compliance with all relevant standards and regulations
