Position Overview
The Rounding RN serves as a consistent care coordinator for patients admitted to the hospital, bridging the gap between inpatient and outpatient care. Using critical thinking skills, the Rounding RN assesses physical, emotional, and social needs while supporting informed decision-making and improving overall health, treatment, and end-of-life outcomes. This role facilitates access to quality care, decreases barriers, and enhances the patient experience through seamless communication across the healthcare team.
Key Responsibilities
- Perform purposeful patient rounding with or without the provider
- Assess, plan, facilitate, coordinate, and evaluate care to achieve quality, cost-effective outcomes
- Complete documentation for provider review and enter orders as directed in the electronic health record
- Create a seamless flow of care across settings to reduce barriers and improve outcomes
- Provide education and anticipatory guidance to patients, families, and caregivers
- Develop and coordinate resources, education, and activities that support patients and families
- Maintain reliable systems to document, track, and monitor patient data
- Perform RN-level telephone duties and follow-up care coordination
- Collaborate with oncologists to track patient progress and adjust plans of care as needed
- Coordinate patient appointments, specialist consultations, and outside services to enhance care
- Support patients in managing conditions, prioritizing healthcare needs, and preventing complications
- Work with insurance, community resources, integrative oncology, and supportive services
- Educate patients on prevention, symptom management, and individualized care plans to reduce admissions/readmissions
- Partner with clinical teams to provide value-based nursing services
- Perform other job functions as assigned
Qualifications
Education & Experience
Required:
- Graduate of an accredited nursing program
- Minimum one (1) year of nursing experience
- Current RN license (state or compact license)
- BLS Certification (required for all new hires)
Preferred:
- Bachelor’s degree in Nursing
- Outpatient nursing and scheduling experience
