The Licensed Practical/Vocational Nurse (LPN/LVN) provides direct patient care under the supervision of a Registered Nurse (RN). In this role, you will follow individualized care plans, provide treatments, and collaborate closely with the interdisciplinary team to achieve positive patient outcomes, with a focus on end-of-life care.
What You'll Do
- Deliver direct patient care in accordance with the State Nurse Practice Act, including specialized end-of-life care duties.
- Implement current nursing practices based on comprehensive patient assessments and established care plans.
- Maintain accurate, timely, and thorough documentation of patient care to reflect adherence to the plan of care.
- Coordinate hospice services and communicate patient needs effectively to the interdisciplinary team, including hospice aides, occupational/physical/speech therapists, social workers, dieticians, and clinical supervisors.
- Provide appropriate pain and symptom management, evaluating patient responses to treatments and medications.
- Ensure clinical documentation reflects ongoing communication and coordination with physicians, facility staff, and team members.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Currently licensed as an LPN/LVN in the State
- Minimum one (1) year experience as a nurse in a clinical care setting
Desired Qualifications
- Hospice or home health environment preferred

- Service Area: Wyoming, Caledonia, Lowell, and surrounding communities
Schedule
- Regular Hours: 8:00 AM–5:00 PM, Monday through Friday
- Weekend Rotation: Required on rotating basis
Pay Range
$24 - $28 USD
