About the Role

Lead a dedicated team of healthcare professionals in delivering exceptional home health care. As a Home Health Clinical Manager, you'll oversee all aspects of patient care service - from admission through discharge - while developing and mentoring clinical staff, ensuring regulatory compliance, and building relationships with referral sources that drive quality patient outcomes.  This leadership role is perfect for an experienced RN or PT who wants to leverage clinical expertise in a management capacity. You'll balance strategic oversight with hands-on clinical guidance, ensuring that every patient receives coordinated, high-quality care while supporting your team's professional growth and success.

What You'll Do

Provide Clinical Leadership and Oversight

  • Oversee all patient care services for your assigned team, ensuring quality and continuity
  • Provide expert clinical guidance on complex cases and challenging situations
  • Ensure coordination of care through oversight of professional practice standards
  • Conduct supervisory visits and consultations to advance care knowledge
  • Maintain availability during operating hours to support patient care needs and staff questions

Lead and Develop Your Team

  • Supervise RNs, PTs, OTs, MSWs, and home health aides, providing clinical direction and support
  • Conduct performance appraisals and provide coaching based on needs assessments
  • Participate in hiring and orientation of new clinical personnel
  • Meet regularly with field staff to provide guidance on clinical and operational issues
  • Identify training needs and coordinate in-services and educational opportunities
  • Address performance issues and provide corrective action when needed

Ensure Quality and Compliance

  • Ensure compliance with state licensure, Medicare certification, and accreditation standards
  • Monitor OASIS assessments and care plan development for accuracy and completeness
  • Implement QAPI recommendations to improve patient care and operations
  • Coordinate utilization review activities to optimize care delivery
  • Stay current with Medicare, Medicaid, and third-party payor regulations
  • Develop and implement policies and procedures that support regulatory compliance

Coordinate Patient Care and Referrals

  • Oversee patient and personnel assignments to ensure appropriate care delivery
  • Ensure plans of care are developed, implemented, and updated based on patient needs
  • Collaborate with physicians, hospitals, SNFs, and case managers on care coordination
  • Consult with QA Specialists on complex care plans and documentation
  • Ensure staff availability and competency to respond to referrals promptly
  • Coordinate services with providers, vendors, and facilities

Manage Operations and Resources

  • Monitor employee overtime, absenteeism, and scheduling efficiency
  • Provide input to administrative and EMR systems related to patient care
  • Coordinate weekend on-call services provided by staff
  • Assist business development team with coordinated customer efforts
  • Report accidents, incidents, or unusual occurrences immediately
  • May serve as backup for other clinical managers as needed

Build Strategic Relationships

  • Maintain professional relationships with referral sources including hospitals and physicians
  • Represent the organization in marketing and public relations activities
  • Ensure customer expectations are met through responsive, quality care delivery
  • Support growth initiatives through clinical excellence and relationship building

What We're Looking For

Required Qualifications

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing OR Licensed Physical Therapist
  • Current State License (RN or PT)
  • Reliable transportation with valid automobile insurance
  • Availability during all operating hours with flexibility for schedule disruptions

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) for RN candidates
  • 2-3 years of home health experience
  • Previous supervisory or management experience

Essential Skills and Competencies

  • Excellent observation, verbal, and written communication skills
  • Strong clinical judgment and ability to provide expert guidance on complex cases
  • Knowledge of business management and healthcare operations
  • Understanding of governmental regulations, Medicare/Medicaid requirements, and CHAP standards
  • Proficiency with home health EMR systems
  • Ability to assess and respond to varied patient, family, and staff needs
  • Strong leadership and personnel management capabilities
  • Ability to handle multiple priorities, deadlines, and frequent disruptions
  • Cultural sensitivity and tolerance for diverse lifestyles
  • Conflict resolution and problem-solving skills

Work Environment and Schedule

This position is based in our office with minimal travel for marketing activities, public relations, and occasional patient home visits. The role requires high flexibility - you must be available during all operating hours and responsive to urgent clinical and operational needs.  Frequent disruptions to routine are expected as you respond to patient emergencies, staff questions, referral source requests, and operational challenges. Occasional long hours may be necessary during high-census periods or staffing challenges. Weekend on-call coordination is part of the role.

Pay Range

$93,500 - $104,000 USD
