Senior Program Manager
Akido is rebuilding the healthcare experience from the ground up. Through early interventions designed around social determinants of health (SDoH), world-class care focused on chronic disease, and human-centered technology, we believe we can build a healthcare model that allows for all patients to live their fullest lives. We are a fast-growing, impact-focused, Y Combinator company created out of the University of Southern California’s D-Health Lab with the idea that empowering government, healthcare, and nonprofit services with population-based data could help usher in a new era of preventive public health. Today we are building a full stack medical network that leverages our predictive capabilities to provide a frictionless experience for both our patients and care providers.
The Opportunity
At Akido Care, we’re redefining healthcare for underserved communities by addressing the needs of populations with complex medical and social challenges. As a Senior Program Manager, you’ll be a player-coach: you’ll continue leading your own pod as well as elevate outcomes across other pods by coaching/mentoring Program Managers and jumping in to stabilize operations when needed. You’ll help us deliver compassionate, high-quality care at scale for Medi-Cal members, people experiencing homelessness, and individuals with behavioral health and substance use needs.
What you’ll do
- Onboard and coach new Program Managers to help them succeed in their first 90 days. Mentor new Program Managers across areas such as people management, and performance management.
- Represent Akido to external partners in the absence of Director.
- Manage a caseload of patients. This includes completing assessment forms with them, developing care plans for patients (with patients and clinical teams), and carrying out activities according to the care plan.
- Build rapport with patients with a goal of increasing the likelihood of positive behavior changes.
- Coach patients to minimize risks associated with the identified common health conditions and behaviors.
- Accompany members to medical and social services appointments.
- Connect members to appropriate programs to address barriers to care and to enhance compliance.
- Link members to local, county and state services. Follow up with members and serve as a member advocate.
- Introduce systems to promote self-management & self-efficacy.
- Document information from every encounter in designated information systems.
- Outreach and engage with eligible patients to enroll them into the Akido ECM Program.
- Other duties as assigned.
Who you are
- Demonstrated traits of a successful Program Manager at Akido, including role modeling core values (empathy, respect, teamwork), patient centered care, people management.
- Deep understanding of Medi-Cal populations, including those experiencing homelessness, mental illness, or substance use disorders.
- 4+ years experience managing front line staff
- Exceptional organizational and time-management abilities.
- Strong interpersonal and communication skills, including team coaching and conflict resolution.
- Familiarity with compliance standards, documentation systems, and data-driven decision-making.
