The Opportunity
Join us in tackling healthcare for rural America! The Michigan State Medical Director will play a key role in leading direct patient care and enabling local providers to succeed in a value-based care model. You will lead clinical operations, support local health systems,, and work closely with Product, Operations, and Clinical Strategy to ensure the effective implementation of our mission to provide high-quality, accessible care to rural populations. This hybrid role combines leadership with hands-on medical practice, making a direct impact on patient outcomes while building the capacity of the local healthcare ecosystem.
What You’ll Do:
- Lead Patient Care
- Provide compassionate, high-quality care to patients in rural settings, including telemedicine and in-person visits.
- Be a leader in a multi-disciplinary care model that leverages community health workers, nurses, providers, pharmacists, and many other team members
- Ensure care is aligned with value-based care principles, focusing on preventive health, chronic disease management, and patient engagement.
- Lead the clinical integration of innovative care delivery models such as virtual health, home-based care, and mobile health services.
- Clinical Leadership & Enablement
- Serve as a clinical partner to local healthcare providers, offering specific action plans and care coordination for shared patients
- Promote collaboration between primary care providers, specialists, and other healthcare professionals to enhance care delivery and patient outcomes.
- Operational Oversight & Quality Improvement
- Oversee clinical operations in Homeward’s largest region, ensuring high standards of care, regulatory compliance, and alignment with company goals.
- Use data analytics to monitor population health trends, improve clinical outcomes, and identify areas for continuous improvement.
- Drive quality initiatives aimed at improving patient care and reducing unnecessary healthcare costs, including managing high-risk and rising-risk populations.
- Community & Stakeholder Engagement
- Build and maintain relationships with key stakeholders, including local health systems, community leaders, and other healthcare organizations.
- Engage with patients and families to understand their unique needs and incorporate feedback into care delivery strategies.
What You Bring:
- MD or DO with an active medical license; board certification in family medicine, internal medicine, or another primary care specialty strongly preferred
- Must be licensed in Michigan and ideally Minnesota, preferably both
- 8+ years of clinical experience, with increasing leadership roles, including experience in rural or underserved settings
- Proven experience in value-based care, care management, or population health management
- Strong leadership and team-building skills, with a track record of mentoring and supporting healthcare providers
- Excellent communication, interpersonal, and collaboration skills.
- Occasional travel to meet with providers across the region
Bonus Points:
- Experience in a healthcare leadership role in a value-based care setting
- Knowledge of rural health systems and the unique challenges faced by rural healthcare providers and patients
- Experience in data-driven care models, including the use of population health tools and electronic medical records (EMRs)
