Licensed Master's Social Worker - Wolf River
Position Summary: This position provides assistance to patients and families in identifying social, emotional, environmental and financial needs. The position will address these needs as a direct resource, or as a referral agent, that educates and connects the individuals with available resources and services within the Clinic or community. The position will address the psychosocial needs of individuals through assessments, interventions and counseling in collaborations with the care team. Work is performed under the general direction of the Director, Wings Supportive Care Division.
Essential Duties & Responsibilities:
- Gathers and assesses information regarding the patient’s physical needs, mental status, family support, financial resources, and potential barriers to care.
- Develops a personalized plan which identifies and addresses patient’s needs - in coordination with patient care support team, care providers and the patient.
- Conducts psychosocial assessments of patients and families and develops plans to address needs.
- Provides direct interventions to patients and their family/caregivers including individual, family and/or group counseling and crisis intervention as needed.
- Remains current on available resources in community and governmental agencies, referring patients to programs as needed.
- Provides psycho/social support to patient and family.
- Acts as advocate for patient and family.
- Maintains appropriate documentation and statistical data.
- Respects the dignity and confidentiality of patients.
- Maintains a good attendance record and reports on-time for work.
- Maintains a professional attitude and appearance.
- Possesses good verbal and written communication skills that lend to a team-oriented work environment.
- Attends staff meetings as scheduled.
- Interfaces with Physicians and Nurses concerning psycho/social health issues and patient/family interaction. Also interfaces with community for various community programs.
- Some travel may be required.
- Performs other duties as assigned.
Qualifications/Experience:
Master’s degree from an accredited school of Social Work. One year of professional practice post-graduation. Experience in Oncology or a Healthcare setting preferred. Must be a Licensed Master Social Worker.
Knowledge of the following areas is beneficial: Oncology disease processes and its effects on patients and families; insurance, state and federal, regulations and resources specific to oncology; social work theory, crisis intervention and individual/group/family therapy.
Must have excellent organization and follow-up skills; excellent verbal and written communication skills; excellent problem-solving skills; ability to organize and prioritize work assignments; ability to handle multiple priorities in a fast-paced environment; ability to analyze situations and respond in a timely manner; ability to participate in multi-functional teams; ability to establish and maintain effective working relationships within The West Clinic. Must be proficient in computer applications including word processing and e-mail. Additional hours may be required.
Work Environment/Physical Demands/Travel:
- Typical clinical environment with moderate noise level.
- Ability to walk, sit and use hand/wrist, stoop or crouch, talk, hear, see for extended periods of time.
- Ability to add, subtracts, multiply, and divide in all units of measure, using whole numbers, fractions and decimals. Ability to compute rate, ratio, and percent and to interpret bar graphs.
- Ability to lift up to 10lbs.
