We are currently looking for a compassionate, organized, and team-oriented Home Health Physical Therapist (PT) to serve patients throughout San Francisco. In this essential role, you will collaborate closely with our interdisciplinary team, including nurses, therapists, and administrative staff, to provide individualized care and ensure high-quality patient outcomes.

Responsibilities

• Provide physical therapy services within the scope of practice, following individualized plans of care

• Deliver treatment, evaluate patient response, and report changes to the Physician, Case Manager, and/or Director of Nursing

• Maintain appropriate clinical records and submit documentation on company devices per organizational policy

• Supervise PTAs if assigned

• Follow all company policies and procedures

• Perform other duties as assigned

Requirements

• Current California PT License (required)

• Minimum 1 year of home health experience preferred

• Valid California Driver’s License

• Ability to travel within San Francisco and surrounding areas

• Caring, compassionate personality with strong communication skills

Job Type: Full-Time / Per Diem

Pay: $70–$90/hour DOE
