We are strengthening our Santa Clara County team and are seeking a Physical Therapist who wants flexibility, independence, modern tools, and an organized home health structure behind them.

Compensation & Benefits

• $75–$90/hour (depending on experience)

Your Role

• Independently perform physical therapy evaluations and reassessments

• Develop and manage individualized plans of care

• Deliver skilled therapy focused on mobility, function, fall prevention, and independence

• Supervise and collaborate with PTAs as applicable

• Communicate directly with physicians and the interdisciplinary team

• Educate patients and caregivers

• Complete documentation in Kinnser supported by AI-assisted charting tools

• Serve as the primary PT voice in patient care planning

South Bay / Santa Clara County 

 

Requirements

• Current California Physical Therapist license

• Home health experience preferred; strong outpatient, inpatient, or rehab PTs encouraged to apply

• Valid California driver’s license 

• Strong clinical judgment, organization, and time-management skills
