Health Link Home Health and Hospice has been a trusted provider of comprehensive home health services throughout the greater San Francisco Bay Area and East Bay for nearly two decades. We are expanding our team and are seeking compassionate, organized, and team-oriented Occupational Therapists (OTs) for multiple territories.

Available Territories & Pay Structure

We are hiring for three separate territories. Compensation is based on experience and territory assignment:

• Alameda County & West Contra Costa County
  Pay Rate: $60–$70 per hour DOE - Eligibility of a $10,000 Sign on Bonus for Full-Time status employees
  Coverage Area: Oakland/San Leandro/Berkeley/Richmond/San Pablo
  
  
• East Contra Costa County
  Pay Rate: $60–$70 per hour DOE
  Coverage Area: Antioch/Pittsburg/Brentwood
  
  
• TriValley Area
  Pay Rate: $60–$70 per hour DOE
  Coverage Area: Dublin/Livermore/Pleasanton/San Ramon/Danville

Bonus Information

Health Link’s bonus structure is quality-driven—not volume-driven.

Responsibilities

• Provide Occupational Therapy services according to the individualized plan of care
• Deliver treatments, evaluate patient response, and report condition changes to the Case Manager, Physician, and/or Director of Nursing
• Maintain accurate and timely clinical documentation
• Adhere to all company policies and procedures
• Collaborate effectively with the interdisciplinary team to ensure optimal patient outcomes

Requirements

• Current California OT License (Required)
• Home Health Experience: 1 year (Preferred)
• Current CA Driver’s License
• Excellent organizational and communication skills
• Ability to work independently in the field
