We are currently seeking an experienced, clinically confident Home Health Registered Nurse (RN) – Level 2 to serve patients throughout the San Mateo Peninsula. In this role, you will manage a consistent caseload with increased autonomy while collaborating closely with physicians, therapists, and interdisciplinary team members to ensure excellent patient outcomes.

Responsibilities

• Perform comprehensive nursing assessments and manage ongoing home health care

• Deliver skilled nursing services including medication management, wound care, chronic disease management, and patient education

• Communicate changes in patient condition to Physicians, Case Managers, and the Director of Nursing

• Maintain accurate, timely clinical documentation on company-issued devices

• Coverage Area: Fixed, compact territory to minimize drive time

• Follow all company policies, clinical standards, and regulatory requirements

• Perform other duties as assigned

Requirements

• Current California RN License (required)

• 1–3 years of clinical nursing experience; Home Health experience preferred

• Valid California Driver’s License

• Ability to travel throughout the San Mateo Peninsula and surrounding areas

• Strong clinical judgment, organization, and communication skills

Job Type: Full-Time / Per Diem

Pay: $69.00 – $72.00 per hour

Annual Full-Time Expectation: $144,000 – $150,000 per year
