
Hospice Social Worker (South Bay)

We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Part-Time or Per Diem Hospice Social Worker for the city of San Francisco and the Peninsula and all surrounding regions.
In this essential position, you will work closely with our Director of Nursing, clinical staff, and front office team to ensure coordinated patient care and provide support to our exceptional team of healthcare professionals. Your contributions will help us maintain our reputation for excellence and enhance the patient experience.

Duties & Responsibilities:
- Evaluates the need for medical social casework services to patients in their place of residence.
- Communicates patient needs to the physician and hospice interdisciplinary team.
- Plans quality medical social work services.
- Implements social work services to patient and their families in their place of residence.
- Evaluates medical social work services plan on an on-going basis.
- Maintains assessment and progress documentation accurately.
- Attends and presents appropriate patient-related information at the Interdisciplinary Team Meetings
- Participates in the quality improvement program.
Experience and Education
- Master of Social Work degree from a school accredited by the Council on Social Work Education
- One year social work experience in a health care setting required
- Clinical experience with geriatric, or terminally ill populations desirable
- Current CPR certification
