Clinical Support Nurse
What will you be doing:
We are looking for an experienced, talented, and compassionate Clinical Support Nurse for a full-time Monday - Friday position for our Norwell Office.
The Clinical Support Nurse provides comprehensive support across triage, telemedicine, and care coordination functions. This role ensures timely, safe, and efficient patient care by collaborating with clinicians, managers, and support staff. The nurse will assist with live triage, telemedicine monitoring, patient education, and scheduling coordination.

Triage & Communication
- Answer live triage and physician lines; appropriately route calls and information to clinicians and managers.
- Make wellness calls for appointment reminders, follow-ups, and patient education.
- Coordinate and document lab results; distribute to appropriate parties.
- Triage patient and employee concerns and escalate as needed.
- Ensure timely communication between physicians, clinicians, and patients.
Telemedicine Support
- Conduct in-home and virtual visits to install and educate patients on telemedicine equipment.
- Monitor biometric data daily; report findings to clinicians or physicians.
- Troubleshoot telemedicine equipment remotely and in-person.
- Manage telemedicine referrals and ensure complete documentation.
- Maintain inventory and cleanliness of telemedicine equipment.
Care Coordination & Scheduling
- Collaborate with managers and CTAs to support patient admissions and clinician coverage.
- Assist with same-day sick calls, vacation coverage, and scheduling adjustments.
- Support documentation of verbal orders and ensure timely delivery of clinical information.
Compliance & Quality
- Ensure agency compliance with federal/state laws and corporate policies.
- Maintain quality standards and participate in policy development.
- Perform other duties as assigned.
What we’re looking for in you:
Qualifications

- Current MA RN or LPN License in good standing
- Current CPR certification
- Graduate of an accredited RN or LPN program
Experience
-
- Minimum 2 years of direct patient care experience
- Home care and triage experience strongly preferred
- Experience in telemedicine and community health is a plus
- Knowledge of Medicare/Managed Care preferred

Compensation: 36/hr. - 50/hr. Based discipline and on experience
