Job Purpose: 

The Shared Living Coordinator (SLC) at Aspire Living & Learning plays a vital role in the support and oversight of Home Care Providers to ensure the overall health and safety of the individuals supported in Shared Living arrangementsThe SLC collaborates with the Aspire management team, coworkers, state agencies, family members, guardians, medical/mental health providers and others in the community to make sure individuals are safe, their needs are met, and their lives are as rich and fulfilling as possible 

 

Key Responsibilities: 

 

The SLC is responsible to monitor the Providers to assure for programmatic and operational management of services that will enable individuals to reach their fullest potential, ensuring that all individuals are afforded opportunities to experience community activities, develop relationships, make individual choices and decisions, learn new skills in all levels of living, and are treated with dignity and respect. Below outlines key aspects of the roles and responsibilities this position holds: 

 

Relationships and Communication: 

  • Creates positive relationships with all internal and external stakeholders including employees, peers, local and state agencies as well as parents and guardians  
  • Ensures all communication is timely and occurs in a manner that is professional, clear and concise  
  • Builds trust, mutual respect and cohesion around a shared mission  

Program Management: 

  • Provides ongoing support of the Home Care Providers (HCPs) including assisting with the selection of new HCPs, ensuring active HCPs are current with training and certifications and coordinating routine/preventative medical care for supported individuals.  
  • Routinely make home visits, complying with regulatory guidance along with conducting necessary home study evaluations and recertifications of homes when required.  
  • Ensures adherence to organization-wide policies and procedures and state and federal regulations, meeting licensing and certification standards  
  • Champions an environment in which supported individuals have choices, are treated with respect and dignity and actively engaged in their community.  
  • Provides ongoing developmental support of the HCP, ensuring they are meeting ongoing programmatic requirements.  
  • Coordinates services to provide medical, psychiatric and social-emotional needs of the supported individuals  

Leadership and Coordination: 

  • Fosters an environment where the dignity, decisions and rights of the individuals are valued.  
  • Coordinate with and provides support to other departments such as Quality Assurance/Quality Enhancement, nursing, clinical and other administrative departments. 
  • Foster a positive work environment while supporting a culture of teamwork, growth, collaboration and communication. 
  • Cultivate a positive and productive workplace atmosphere to enhance team morale and productivity. 
  • Participate in and complete training sessions as delegated by management to enhance skills and knowledge relevant to the role. 
  • Conduct training as required and assigned.  
  • Perform any additional job responsibilities assigned by supervisors or team leaders to support the organization's smooth operation. 
  • Accept reassignment as required. 

 

Qualifications: 

  • Bachelor’s degree or equivalent years of experience 
  • Excellent interpersonal skills, organizational, administrative and communication skills 
  • Knowledge of funding agency regulations/requirements 
  • Must have demonstrated computer literacy 
  • 18 years of age or older  
  • Ability to provide on-call support 
  • Must have satisfactory background checks  
  • Valid driver’s license and safe driving record 

 

Working Conditions: 

  • Primary work is done within an office setting, however it is expected that the employee will visit program operations as required to fulfill their job duties.  
  • May be required to travel including infrequent day and/or overnight travel  
  • Some holiday work required to meet on call schedule as needed.  
  • Lift/carry up to 10 pounds; speaking; standing; sitting for long periods of time; reaching; bending; walking; reaching with arms and hands, balancing; using fingers to handle or feel.  Vision abilities required by this job include close vision. 
  • This position requires the routine use of standard office equipment such as computers, phones, copiers, fax machines, audio/visual equipment, etc.

Pay: $58,000/YR
